自拟消炎利胆汤对急性梗阻性化脓性胆管炎患者术后肝功能水平的影响

摘　　要：目的探讨自拟消炎利胆汤对急性梗阻性化脓性胆管炎(AOSC)患者术后肝功能水平的影响。方法选取91例2019年6月—2020年2月治疗的AOSC患者为研究对象,按照随机数字表法分为对照组(45例)与观察组(46例)。2组均行胆道解除术,对照组于术后给予盐酸莫西沙星片、地塞米松片治疗,观察组在对照组基础上联合自拟消炎利胆汤治疗,7 d为一个疗程,共治疗2个疗程。比较治疗后2组患者临床症状恢复时间,治疗前后肝功能及血清IL-6、IL-10水平的变化。结果治疗后,观察组腹痛缓解时间、体温恢复时间、黄疸减轻时间相比于对照组明显缩短;与治疗前相比,治疗后2组患者血清ALT、D-Bil和I-Bi及血清IL-6均降低,且观察组显著低于对照组,血清IL-10水平均升高,且观察组高于对照组(均P<0.05)。结论自拟消炎利胆汤可有效缓解AOSC患者术后的临床症状,改善患者肝功能,降低机体炎症反应。Objective To investigate the effect of self-made Xiaoyan Lidan decoction on liver function levels in patients with acute obstructive suppurative cholangitis.Methods A total of 91 patients with acute obstructive suppurative cholangitis in our hospital from June 2019 to February 2020 were selected as the research subjects,and were divided into the control group(n=45)and the observation group(n=46)according to a random number table.Biliary tract discharge was performed in both groups,while moxifloxacin hydrochloride tablets and dexamethasone tablets were given to the control group after surgery,and the observation group was treated with self-made Xiaoyan Lidan decoction on the basis of the control group.7 days was a course of treatment,with a total of 2 courses.After treatment,the recovery time of clinical symptoms,changes in liver function and serum IL-6 and IL-10 levels were compared between the two groups.Results After treatment,the time to relieve abdominal pain,body temperature recovery,and jaundice in the observation group were shorter than those in the control group.Compared with before treatment,the serum ALT,D-Bil and I-Bi and serum IL-6 all decreased,the observation group was lower than the control group,while the serum IL-10 level increased,and the observation group was higher than the control group(all P<0.05).Conclusion The self-made Xiaoyan Lidan decoction can effectively relieve the postoperative clinical symptoms of patients with acute obstructive suppurative cholangitis,improve the liver function of patients,and reduce the body’s inflammatory response.
