摘  要:目的评价沉默信息调节因子1(SIRT1)在电针减轻大鼠脑卒中后中枢性痛(CPSP)中的作用及其与NOD样受体热蛋白结构域相关蛋白3(NLRP3)的关系。方法SPF级健康雄性SD大鼠50只,6周龄,体重180~220 g,采用随机数字表法分为5组(n=10):假手术组(Sham组)、CPSP组、CPSP+假电针组(SEA组)、CPSP+电针组(EA组)和CPSP+电针+SIRT1抑制剂EX527组(EX527组)。除Sham组外,其余大鼠采用丘脑腹后外侧核内注射Ⅳ型胶原酶制备CPSP模型。于模型制备成功后24 h时EA组电针(2/15 Hz,30 min/d,持续5 d)刺激大鼠内关、人中和三阴交穴;SEA组电针刺激内关、人中和三阴交穴旁开5 mm处,其他处理同EA组;EX527组于电针刺激前30 min腹腔注射SIRT1抑制剂EX5275 mg/kg,其他处理同EA组。于模型制备前1 d(T0)和模型制备后1、3和5 d(T1~3)时测定热缩足潜伏期(TWL)和机械缩足反应阈(MWT),随后处死大鼠取脑组织,采用Western blot法检测SIRT1、NLRP3、IL-18和IL-1β的表达。结果与Sham组比较,CPSP组、SEA组、EA组和EX527组大鼠T1~3时TWL和MWT降低,脑组织SIRT1表达下调,NLRP3、IL-18和IL-1β表达上调(P<0.05);与CPSP组比较,EA组大鼠T1~3时TWL和MWT升高,脑组织SIRT1表达上调,NLRP3、IL-18和IL-1β表达下调(P<0.05),SEA组各指标比较差异无统计学意义(P>0.05);与EA组比较,EX527组大鼠T1~3时TWL和MWT降低,脑组织SIRT1表达下调,NLRP3、IL-18和IL-1β表达上调(P<0.05)。结论SIRT1参与了电针减轻大鼠CPSP的过程,与抑制NLRP3表达有关。Objective To evaluate the role of silent information regulator 1(SIRT1)in electroacupuncture(EA)-induced reduction of central post-stroke pain(CPSP)and the relationship with nod-like receptor pyrin domain containing 3(NLRP3)in rats.Methods Fifty SPF healthy male Sprague-Dawley rats,aged 6 weeks,weighing 180-220 g,were divided into 5 groups(n=10 each)using a random number table method:sham operation group(group Sham),CPSP group,CPSP+sham EA group(group SEA),CPSP+EA group(group EA)and CPSP+EA+SIRT1 inhibitor EX527 group(group EX527).TypeⅣcollagenase was injected into the right ventral posterolateral nucleus to establish the model of CPSP in CPSP,SEA,EA and EX527 groups.At 24 h after the model was established successfully,30 min EA(frequency 2/15 Hz)stimulation of Neiguan,Renzhong and Sanyinjiao was performed once a day for 5 consecutive days in EA group.EA was performed at the points 5 mm lateral to the acupoints of Neiguan,Renzhong and Sanyinjiao in group SEA,and the other procedures were similar to those previously described in group EA.SIRT1 inhibitor EX5275 mg/kg was injected intraperitoneally at 30 min before EA stimulation in group EX527,and the other procedures were similar to those previously described in group EA.At 1 day before the establishment of model(T0)and at 1,3 and 5 days after the establishment of model(T1-3),the thermal withdrawal latency(TWL)and mechanical withdrawal threshold(MWT)were measured.The animals were then sacrificed and brain tissues were taken for determination of the expression of SIRT1,NLRP3 and interleukin(IL)-18 and IL-1β.Results Compared with Sham group,the TWL was significantly shortened and the MWT was decreased at T1-3,the expression of SIRT1 was down-regulated,and the expression of NLRP3,IL-18 and IL-1βwas up-regulated in CPSP,SEA,EA and EX527 groups(P<0.05).Compared with CPSP group,the TWL was significantly prolonged and the MWT was increased at T1-3,the expression of SIRT1 was up-regulated,and the expression of NLRP3,IL-18 and IL-1βwas down-regulated in EA group(P<0.0
