妊娠期高血压疾病糖脂代谢及其与血清Visfatin、RBP4浓度的相关性分析

摘　　要：目的分析妊娠期高血压疾病(HDP)孕妇糖脂代谢状况,并探讨其与血清内脂素(visfatin)、视黄醇结合蛋白4(RBP4)浓度的关系。方法选择2019年1月至2020年6月唐山市妇幼保健院收治的HDP孕妇60例为研究对象,设为观察组。选择同期产检的正常孕妇60例作为对照组,比较两组参与者分娩前糖脂代谢状况及血清visfatin、RBP4浓度,进一步分析HDP患者糖脂代谢指标与血清visfatin、RBP4浓度的相关性。结果观察组空腹胰岛素、稳态模型胰岛素抵抗指数(HOMA-IR)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL)、visfatin、RBP4浓度均显著高于对照组,差异有统计学意义(t值2.5484.456,P<0.05),而两组参与者空腹血糖、高密度脂蛋白(HDL)浓度比较差异无统计学意义(t值分别为0.214、0.828,P>0.05)。HDP患者空腹胰岛素、HOMA-IR与血清visfatin、RBP4浓度均呈显著正相关(r值分别为0.298、0.257、0.301、0.286,P<0.05),而空腹血糖与血清visfatin、RBP4浓度无显著相关性(r值分别为0.074、0.098,P>0.05)。HDP患者TC、TG、LDL与血清visfatin、RBP4浓度均呈显著正相关(r值分别为0.312、0.289、0.379、0.264、0.245、0.238,P<0.05),而空腹HDL与血清visfatin、RBP4浓度无显著相关性(r值分别为-0.097、-0.065,P>0.05)。结论HDP患者易出现糖脂代谢紊乱,且其与血清visfatin、RBP4浓度相关,值得临床关注。Objective To analyze the status of glucose and lipid metabolisms in women with hypertensive disorders of pregnancy(HDP),and to analyze its relationship with serum visfatin and retinol-binding protein 4(RBP4)levels.Methods A total of 60 pregnant women with HDP who were admitted to Tangshan Maternal and Child Health Hospital from January 2019 to June 2020 were selected as the observation group.Sixty cases of normal pregnant women in the hospital during the same period were selected as the control group.The glucose and lipid metabolisms status and serum visfatin and RBP4 concentrations of participants between the two groups before delivery were compared,and the correlations between glucose and lipid metabolisms indexes and serum visfatin and RBP4 concentrations in HDP patients were further analyzed.Results The concentrations of fasting insulin,homeostasis model assessment of insulin resistance(HOMA-IR),total cholesterol(TC),triglyceride(TG),low density lipoprotein(LDL),visfatin and RBP4 in the observation group were significantly higher than those in the control group(t values were 2.548~4.456,all P<0.05).But there was no significant difference in fasting blood glucose and high density lipoprotein(HDL)concentrations between the two groups(t values were 0.214 and 0.828,respectively,P>0.05).There were significant positive correlations between fasting insulin,HOMA-IR and serum visfatin,RBP4 concentrations in HDP patients(r values were 0.298,0.257,0.301,0.286,respectively,P<0.05).There was no significant correlation between fasting blood glucose and serum visfatin,RBP4 concentrations(r values were 0.074 and 0.098,respectively,P>0.05).TC,TG,LDL were significantly positively correlated with serum visfatin and RBP4 concentrations in HDP patients(r values were 0.312,0.289,0.379,0.264,0.245,0.238,respectively,P<0.05).There was no significant correlation between fasting HDL and serum visfatin,RBP4 concentrations(r values were-0.097 and-0.065,respectively,P>0.05).Conclusion Patients with HDP are prone to glucose and lipid meta
