摘　　要：目的提高肾移植术后妊娠患者的用药安全性。方法临床药师参与1例肾移植术后妊娠伴贫血患者的临床治疗,并查阅国内外相关文献,开展药物治疗实践。结果临床药师提出调整优化用药方案,关注血药浓度、疗效及不良反应的建议,提供用药教育,并优化患者的免疫抑制治疗方案。医师采纳药师的建议,该患者的贫血好转。结论临床药师可在肾移植术后妊娠患者的药物治疗中提供合理化建议,对可能出现的不良反应进行监护,并与医师、护士实现三位一体共同协作,为肾移植患者提供全程管理,进一步提升药学服务质量。Objective To improve the medication safety of pregnant patients after renal transplantation.Methods The clinical pharmacist participated in the clinical treatment of a patient with anemia in pregnancy after renal transplantation,and carried out pharmaceutical care by consulting the relevant literature at home and abroad.Results The clinical pharmacist put forward suggestions on adjusting and optimizing the medication plan,paying attention to the plasma concentration,efficacy and adverse reactions,providing medication education for the patient,and optimizing the patient’s immunosuppressive treatment regimen.The physician adopted the pharmacist’s suggestions and the patient’s anemia improved.Conclusion The clinical pharmacist can provide reasonable suggestions in the drug treatment of pregnant patients after renal transplantation,monitor the possible adverse reactions,and coordinate with physicians and nurses to provide whole-process management for transplant patients and further improve the quality of pharmaceutical care.
