摘 要:目的探讨低剂量CT联合体表辅助定位和常规剂量CT经皮肺穿刺活检术在诊断肺部占位性病变中的应用价值和安全性。方法分别收集2016年3月至2018年10月(A组,常规剂量CT)和2018年11月至2020年5月(B组,低剂量CT联合体表辅助定位)进行的320例经皮肺穿剩活检术患者的资料进行回顾性队列研究,对比分析2组诊断敏感度、诊断准确率、手术时间、CT图像质量辐射剂量、并发症发生率,分析其病理类型,随访患者治疗6个月。结果.诊断准确事A组95.76%.B组97.42%,诊断敏感度A组95.51%,B组97.22%,2组间差异无统计学意义(P>0.05)。B组与A组比较,B组具有CT辐射剂量小、手术时间短、CT扫描次数少、并发症少等优点。结论经皮肺穿刺活检术是肺部结节或周围性占位首选的非手术活检方法,实行低剂量CT扫描和辅助引导是经皮肺穿刺活检的发展趋势。Objective To explore the application value and safety of percutaneous lung biopsy under the guidence of low-dose CT combined with surface essisted localization or conventional-dose CT in the diagnosis of pulmonary occupying lesions.Methods A total of 320 patients who required percutaneous lung biopsy underwent either conventiona-dose CT between March 2016 and October 2018(group A)or low-dose CT combined with surface ssisted lcalization between November 2018 and May 2020(group B)were enolled in the retrospective cohort study.The diagnostic sensitivity,diagnostic accuracy rate,operative time,CT image quality,radiation dose,complication rate of the two groups were compared.Pathological types were analyzed and the patients were fllowvewd up for half a year,Results There were no sigificant dfferences between the two groups in the acuracy rates(95.76%vs 97.42%,P>0.05)and diagnostic sensitivityt(95.51%vs 97.22%,P>0.05).Compared with group A,group B had less operation time and CT scan times,lower CT radiation dose,and fewer complications.onclusion Percutaneous lung biopsy is the preferred non-surgical biopsy method for pulmonary nodules or peripheral masses.Auxiliary guided puncture under the guidence of low dose CT scanning is a trend of percutaneous lung biopsy.
