摘 要:目的探讨职业性矽肺(以下简称“矽肺”)患者血清Apelin水平与矽肺分期和肺功能的相关性。方法采用病例-对照研究方法,选择85例男性矽肺患者为矽肺组(其中,矽肺壹期、贰期、叁期患者分别有44、28和13例),以120名无职业病危害因素接触的男性健康人员为对照组。采集2组人群血清样品,以酶联免疫吸附实验法检测Apelin水平;对矽肺组患者进行肺功能检查。结果对照组、矽肺组人群血清Apelin水平中位数与第25、75百分位数[M(P25,P75)]分别为1.29(0.92,1.77)和0.80(0.62,1.04)mg/L。矽肺壹期、贰期、叁期患者血清Apelin水平M(P25,P75)分别为1.03(0.82,1.31)、0.66(0.60,0.80)、0.50(0.30,0.65)mg/L。多元线性回归分析结果显示,在排除年龄和吸烟的影响后,矽肺组人群血清Apelin水平低于对照组(P<0.01)。随着矽肺分期的增加,矽肺组人群血清Apelin水平下降(P<0.001)。矽肺组患者血清Apelin水平与肺活量、用力肺活量、第一秒用力呼气容积、最大呼气中段流量均呈正相关(P值均<0.05)。结论矽肺患者血清Apein水平越低,其病情越严重,肺功能损害越严重。Apelin可能在矽肺的诊断、分期、疗效判定和预后评估中具有一定的意义,并可作为矽肺的潜在治疗靶点。Objective To explore the association of serum Apelin level,silicosis stage and lung function in patients with occupational silicosis(hereinafter referred to as silicosis).Methods A case-control study was conducted.A total of 85 patients with silicosis were selected as the silicosis group(44,28 and 13 patients with stageⅠ,ⅡandⅢsilicosis,respectively),and 120 healthy individuals without occupational hazard exposure were selected as the control group.Serum samples were collected from the cases of the two groups and the level of Apelin was determined by enzyme-linked immunosorbent assay.The pulmonary function of the silicosis group was examined.Results The median and the 25th and 75th percentiles[M(P25,P75)]of serum Apelin levels in the control group and silicosis group were 1.29(0.92,1.77)and 0.80(0.62,1.04)mg/L,respectively.The level of serum Apelin M(P25,P75)in stageⅠ,ⅡandⅢsilicosis patients was 1.03(0.82,1.31),0.66(0.60,0.80)and 0.50(0.30,0.65)mg/L,respectively.The results of multiple linear regression analysis showed that the level of serum Apelin in the silicosis group was higher than that in the control group after excluding the influence of age and smoking(P<0.01).The level of serum Apelin decreased with the increase of silicosis stage in the silicosis group(P<0.001).Serum Apelin level in silicosis group was positively correlated with lung vital capacity,forced vital capacity,forced expiratory volume in the first second,and forced expiratory flow between 25%and 75%(all P<0.05).Conclusion The lower level of serum Apein in silicosis patients,the more serious the disease and the more serious the damage to lung function.Apelin is of significance in the diagnosis,staging,treatment appraisal and prognostic evaluation of silicosis,and it can be use as a potential therapeutic target for silicosis.
