单臂多功能外固定支架治疗胫腓骨骨折的疗效分析

摘　　要：目的分析单臂多功能外固定支架治疗胫腓骨骨折患者的临床疗效。方法83例胫腓骨骨折患者,均采用单臂多功能外固定支架治疗,统计骨折愈合时间、治疗效果及并发症发生情况等。结果83例患者经过随访6~18个月,平均随访时间10个月,骨折愈合时间为3~10个月,平均骨折愈合时间5.6个月。治疗效果优59例,良18例,中5例,差1例,优良率为92.77%。骨折无骨不连、延迟愈合、畸形愈合等,其中3例开放性骨折发生切口浅表感染,皮缘坏死;针道发生炎症13例,经换药治疗后治愈。无深部感染及骨髓炎发生。结论单臂多功能外固定支架在治疗胫腓骨骨折,特别是伴有严重下肢软组织缺损、骨缺损的患者,为骨折愈合提供了良好的生物学和生物力学环境,为各种皮瓣修复提供了便利条件,其手术创伤小,操作简单,有利于骨折愈合和早期功能锻炼,优于其他固定方式,是一种可靠的手术方法。Objective To analyze the clinical effect of single-arm multifunctional external fixator in the treatment of tibia and fibula fractures.Methods All 83 cases of tibia and fibula fractures were treated with a single-arm multifunctional external fixator,and the healing time of the fracture,the therapeutic effect and the occurrence of complications were counted.Results 83 patients were followed up for 6-18 months,with an average follow-up time of 10 months,the fracture healing time was 3-10 months,with an average fracture healing time of 5.6 months.The treatment effect was excellent in 59 cases,good in 18 cases,fair in 5 cases,and poor in 1 case.The excellent and good rate was 92.77%.There were no bone discontinuity,delayed healing,or deformed healing in the fractures.3 cases of the open fractures had superficial infection of the incision and necrosis of the skin margin;13 cases had inflammation of the needle tract;they were cured after dressing change.No deep infection or osteomyelitis occurred.Conclusion For patients with tibia and fibula fractures,especially for patients with severe soft tissue defects and bone defects in the lower limbs,single-arm multifunctional external fixator provides a good biological and biomechanical environment for fracture healing and convenient conditions for various flap repair.The surgical trauma is small,the operation is simple,and it is conducive to fracture healing and early functional exercise.It is superior to other fixation methods and is a reliable surgical method.
