沙库巴曲缬沙坦钠片治疗射血分数降低慢性心力衰竭患者的疗效分析

摘　　要：目的探讨沙库巴曲缬沙坦钠片(商品名:诺欣妥)治疗射血分数降低慢性心力衰竭的疗效。方法82例射血分数降低慢性心力衰竭患者,随机分为对照组及观察组,每组41例。对照组使用常规抗心力衰竭药物治疗,观察组在对照组基础上加用沙库巴曲缬沙坦钠片治疗。对比两组的临床疗效、心功能指标[左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)、左室射血分数(LVEF)、左室收缩末期容积(LVESV)、左室舒张末期容积(LVEDV)]、血压[收缩压(SBP)、舒张压(DBP)]、血生化指标[血肌酐(Scr)、血清钾、血清钠]。结果观察组治疗总有效率92.68%高于对照组的73.17%,差异具有统计学意义(P<0.05)。治疗后,观察组LVEDD、LVESD、LVESV、LVEDV小于对照组,LVEF高于对照组,差异有统计学意义(P<0.05)。治疗后,观察组SBP、DBP高于对照组,差异具有统计学意义(P<0.05);两组Scr、血清钾、血清钠比较,差异无统计学意义(P>0.05)。结论射血分数降低慢性心力衰竭在治疗时使用沙库巴曲缬沙坦钠片的疗效更好,可以对肾素-血管紧张素-醛固酮系统起到更有效的抑制作用,减少神经内分泌,抑制心室重构,减轻心肌损伤,具有积极的临床意义。Objective To discuss the efficacy of sacubitril valsartan sodium tablets(trade name:Novartis)in the treatment of chronic heart failure patients with reduced ejection fraction.Methods A total of 82 cases of chronic heart failure patients with reduced ejection fraction were randomly divided into control group and observation group,with 41 cases in each group.The control group was treated with conventional anti-heart failure drugs,and the observation group was treated with sacubitril valsartan sodium tablets based on the control group.The clinical efficacy and cardiac function indexes[left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD),left ventricular ejection fraction(LVEF),left ventricular end-systolic volume(LVLVESV),left ventricular end-diastolic volume(LVLVEDV)],blood pressure[systolic blood pressure(SBP),diastolic blood pressure(DBP)],blood biochemical indicators[serum creatinine(Scr),serum potassium,serum sodium]were compared between the two groups.Results The total effective rate 92.68%in the observation group was higher than 73.17%in the control group,and the difference was statistically significant(P<0.05).After treatment,the LVEDD,LVESD,LVLVESV and LVLVEDV of the observation group were lower than those of the control group,and LVEF was higher than that of the control group.All the difference was statistically significant(P<0.05).After treatment,the SBP and DBP of the observation group were higher than those of the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in Scr,serum potassium and serum sodium between the two groups(P>0.05).Conclusion Sacubitril valsartan sodium tablets has good efficacy on chronic heart failure patients with reduced ejection fraction,and it can effectively inhibit the renin-angiotensin-aldosterone system,reduce neuroendocrine,inhibit ventricular remodeling,and reduce myocardial damage,which has positive clinical significance.
