Efficacy of low-dose methylprednisolone combined with respiratory stimulant in the treatment of chronic obstructive pulmonary disease complicated with respiratory failure

摘  要:目的分析小剂量甲强龙联合呼吸兴奋剂治疗慢性阻塞性肺疾病合并呼吸衰竭的疗效。方法60例慢性阻塞性肺疾病合并呼吸衰竭患者,采用数字表随机分为对照组及观察组,每组30例。对照组患者给予呼吸兴奋剂治疗,观察组在对照组的基础上增加小剂量甲强龙治疗。比较两组患者治疗前后的动脉血气指标[动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO2)]及肺功能指标[用力肺活量(FVC)、第1秒用力呼气容积(FEV1)],治疗效果,不良反应发生情况。结果治疗前,两组患者的PaO2、PaCO2、FVC、FEV1比较,差异均无统计学意义(P>0.05);治疗后,两组患者的PaO2、FVC、FEV1均较本组治疗前升高,PaCO2均较本组治疗前降低,且观察组患者的PaO2(84.81±7.34)mm Hg(1 mm Hg=0.133 kPa)、FVC(3.71±1.13)L、FEV1(3.50±0.34)L高于对照组的(80.14±6.30)mm Hg、(2.44±1.11)L、(2.44±0.24)L,PaCO2(32.44±3.23)mm Hg低于对照组的(42.44±4.56)mm Hg,差异均具有统计学意义(P<0.05)。观察组总有效率93.33%高于对照组的70.00%,差异具有统计学意义(P<0.05)。两组患者的不良反应发生率比较,差异无统计学意义(P>0.05)。结论小剂量甲强龙联合呼吸兴奋剂治疗慢性阻塞性肺疾病合并呼吸衰竭有利于改善患者的动脉血气指标和肺功能指标,提高治疗效果。Objective To analyze the efficacy of low-dose methylprednisolone combined with respiratory stimulant in the treatment of chronic obstructive pulmonary disease complicated with respiratory failure.Methods A total of 60 patients with chronic obstructive pulmonary disease and respiratory failure were divided into control group and observation group according to random numerical table,with 30 cases in each group.The control group was treated with respiratory stimulant,and the observation group was treated with low-dose methylprednisolone on the basis of the control group.The arterial blood gas indexes[arterial partial pressure of oxygen(PaO2),arterial partial pressure of carbon dioxide(PaCO2)]and pulmonary function indexes[forced vital capacity(FVC),forced expiratory volume in the 1st second(FEV1)]before and after treatment,therapeutic effect,and occurrence of adverse reactions were compared between the two groups.Results Before treatment,there was no statistically significant difference in PaO2,PaCO2,FVC and FEV1 between the two groups(P>0.05).After treatment,the PaO2,FVC and FEV1 of the two groups were higher than those before treatment,and the PaCO2 was lower than that before treatment;the PaO2(84.81±7.34)mm Hg(1 mm Hg=0.133 kPa),FVC(3.71±1.13)L and FEV1 (3.50±0.34) L of the observation group were higher than (80.14±6.30) mm Hg, (2.44±1.11) L and (2.44± 0.24) L of the control group, and the PaCO2 (32.44±3.23) mm Hg was lower than (42.44±4.56) mm Hg of the control group. All the difference was statistically significant (P<0.05). The total effective rate 93.33% of the observation group was higher than 70.00% of the control group, and the difference was statistically significant (P<0.05). There was no statistically significant difference in incidence of adverse reactions between the two groups (P>0.05). Conclusion Combination of low-dose methylprednisolone and respiratory stimulant is beneficial to improve arterial blood gas index and pulmonary function index, and improve the therapeutic effect.
