内科胸腔镜下治疗联合胸腔内注入尿激酶治疗结核性包裹性胸膜炎的疗效观察

摘　　要：目的探讨内科胸腔镜下治疗联合胸腔内注入尿激酶治疗结核性包裹性胸膜炎的疗效。方法113例结核性包裹性胸膜炎患者,按照患者意愿将行内科胸腔镜检查的患者作为治疗组(49例),不同意内科胸腔镜检查的患者作为对照组(64例)。治疗组采取内科胸腔镜下治疗联合胸腔内注入尿激酶治疗,对照组采取在B超引导下抽取胸腔积液并注入尿激酶治疗。观察两组患者胸膜厚度。结果治疗半年后、1年后,治疗组的胸膜厚度分别为(1.20±0.14)、(0.78±0.10)mm,均薄于对照组的(1.73±0.22)、(1.24±0.10)mm,差异有统计学意义(P<0.05)。两组治疗1年后的胸膜厚度均薄于治疗半年后,差异有统计学意义(P<0.05)。结论内科胸腔镜下治疗联合胸腔内注入尿激酶治疗结核性包裹性结核性胸膜炎的方法,疗效确切,可明显减少胸膜肥厚和粘连,有临床应用前景。Objective To discuss the efficacy of medical thoracoscopic therapy combined with intrapleural injection of urokinase in the treatment of tuberculous encapsulated pleurisy.Methods In the 113 patients with tuberculous encapsulated pleurisy,the patients who underwent medical thoracoscopy were regarded as the treatment group(49 cases)according to the patient’s wishes,and the patients who disagreed with the medical thoracoscopy were regarded as the control group(64 cases).The treatment group was treated with medical thoracoscopic therapy and intrapleural injection of urokinase,and the control group was treated with extraction of pleural effusion and injection of urokinase injection under B-ultrasound guidance.The pleural thickness of the two groups was observed.Results After half a year and 1 year of treatment,the pleural thickness of the treatment group were(1.20±0.14)and(0.78±0.10)mm,which were thinner than(1.73±0.22)and(1.24±0.10)mm of the control group,and the difference was statistically significant(P<0.05).The pleural thickness of the two groups after 1 year of treatment was thinner than that after half a year of treatment,and the difference was statistically significant(P<0.05).Conclusion Combination of medical thoracoscopic therapy and intrapleural injection of urokinase shows certain efficacy in the treatment of tuberculous encapsulated pleurisy,and it can obviously pleural thickening and adhesion,which is worthy of clinical application.
