慢性肺源性心脏病患者合并心律失常的影响因素分析

摘　　要：目的:分析慢性肺源性心脏病(C P H D)患者合并心律失常的影响因素。方法:选择我院2016年1月—2020年12月收治的518例CPHD患者,计算合并心律失常的发生率,并采用单因素及多因素Logistic回归分析筛选影响C P H D患者合并心律失常的危险因素。结果:有269例C P H D患者发生心律失常,发生率为51.93%。经单因素分析和多因素Logistic回归分析发现,合并肺部感染(OR=16.544)、电解质紊乱(OR=9.602)、低氧血症(OR=7.279)、心力衰竭(OR=5.512)、酸碱失衡(OR=3.254)是导致CPHD合并心律失常的危险因素(P<0.05)。结论:CPHD合并心律失常的发生率较高,合并肺部感染、电解质紊乱、低氧血症、心力衰竭、酸碱失衡是导致C P H D合并心律失常的独立危险因素。Objective To investigate the incidence of arrhythmia in patients with chronic pulmonary heart disease(CPHD),and to explore the related factors.Methods 518 cases of patients with chronic pulmonary heart disease admitted to our hospital from January 2016 to December 2020 were selected as the research subjects.The incidence of complicated arrhythmia was calculated,and the risk factors of complicated arrhythmia in patients with chronic pulmonary heart disease were screened by univariate and multivariate Logistic regression analysis.Results There were 269 patients with chronic pulmonary heart disease combined with arrhythmia,the incidence was 51.93%.Univariate analysis and multivariate logistic regression analysis found that Pulmonary infection(OR=16.544),electrolyte disturbance(OR=9.602),hypoxemia(OR=7.279),heart failure(OR=5.512)and acid-base imbalance(OR=3.254)were independent risk factors for chronic pulmonary heart disease complicated with arrhythmia(all P<0.05).Conclusion The incidence of chronic pulmonary heart disease complicated with arrhythmia is high.Pulmonary infection,electrolyte disturbance,hypoxemia,heart failure and acid-base imbalance are independent risk factors leading to chronic pulmonary heart disease complicated with arrhythmia.
