摘 要:目的:探究血清硒在新生儿缺血缺氧性脑病(HIE)中的表达意义。方法:选择2017年11月—2020年11月于我院接受治疗的60例确诊为HIE患儿为研究组,另选取同期我院50例健康新生儿为对照组,采集两组新生儿血样检测血清硒、血清脑红蛋白(NGB)、肿瘤坏死因子-α(TNF-α)水平并开展组间比较,而后将60例研究组患儿按照病情分组后再次开展上述因子水平比较,最后计算血清硒与NGB、TNF-α相关性,绘制血清硒对HIE诊断ROC曲线,计算曲线下面积(AUC)。结果:(1)研究组患儿血清硒水平明显低于对照组,NGB和TNF-α水平明显高于对照组(P<0.05);(2)重度HIE组患儿血清硒水平明显低于轻度和中度组,NGB和TNF-α明显高于轻度和中度组(P<0.05);(3)Spearman相关性分析显示,血清硒同NGB和TNF-α均呈负相关(r=-0.9714、-0.9669,P<0.05);(4)计算血清硒对HIE诊断AUC为0.9089(95%CI:0.8058~1.0000,P=0.001)。结论:HIE患儿会出现明显的血清硒水平下降情况,其下降程度同NGB、TNF-α等指标明显相关,可以考虑将血清硒水平作为评估HIE临床指标推广应用。Objective:To investigate the significance of serum selenium expression in neonatal hypoxic-ischemic encephalopathy(HIE).Methods:60 cases of HIE in our hospital from November 2017 to November 2020 were selected as the study group,and 50 cases of healthy newborns in our hospital during the same period were selected as the control group.The blood samples of the two groups were collected to detect the levels of serum selenium,serum Neuroglobin(NGB)and tumor necrosis factor-α(TNF-α)and compared between the two groups.Finally,the correlation of serum selenium with NGB and TNF-αwas calculated,the ROC curve of serum selenium for HIE diagnosis was drawn,and the area under the curve(AUC)was calculated.Results:(1)The serum selenium level of the study group was significantly lower than that of the control group,and the levels of NGB and TNF-αwere significantly higher than those of the control group(P<0.05).(2)The serum selenium level of the severe HIE group was significantly lower than that of the mild and moderate groups,and the levels of NGB and TNF-αwere significantly higher than those of the mild and moderate groups(P<0.05).(3)Spearman correlation analysis showed that serum selenium was negatively correlated with NGB and TNF-α(r=-0.9714,-0.9669,P<0.05).(4)The AUC of serum selenium in the diagnosis of HIE was 0.9089(95%CI:0.8058~1.0000,P=0.001).Conclusion:The serum selenium level of HIE children will decrease obviously,and the decrease degree is obviously related to NGB,TNF-αand other indicators,so it can be considered that the serum selenium level can be used as a clinical indicator to evaluate HIE.
