单肺通气时炎性反应机制与预防研究进展

摘　　要：单肺通气一般是对于需要接受开胸手术的患者实行的措施,主要目的是隔离患侧肺,防止液性分泌物流入健侧,可以给手术开展创造完善的视野以方便手术操作,有效减轻手术侧肺损伤,防止两肺间的交叉感染。在单肺通气过程中患者机体内的炎性细胞被激活,产生相关的炎症因子,直接导致患者的肺部出现炎症反应或者是出现其他的并发症疾病,因此临床上单肺通气引发炎症的相关研究成为热点。目前,诸多学者已经探讨分析了单肺通气诱发炎性反应的相关发病机制,以及预防发生的一些具体建议和对策。基于此,本文对近两年单肺通气时炎性反应的发生机制进行了系统分析,并从保护性策略、麻醉方式或者药物、预处理方案等方面阐述单肺通气时炎性反应的相关预防研究,为后期临床需要接受开胸手术的患者提供指导,以降低术后各种不良反应以及并发症发生率。One lung ventilation is generally a measure for patients who need thoracotomy.Its main purpose is to isolate the affected side of the lung and prevent liquid secretions from flowing into the healthy side.It can create a perfect vision to facilitate the operation,effectively reduce the lung injury on the operation side and prevent cross infection between the two lungs.In the process of one lung ventilation,inflammatory cells in patients can be activated to produce related inflammatory factors,which directly lead to inflammatory reaction in patients’lungs or other complications.Therefore,the research on inflammation induced by one lung ventilation has become a hot spot.At present,many scholars have discussed and analyzed the pathogenesis of inflammatory response induced by one lung ventilation,as well as some specific suggestions and countermeasures to prevent the occurrence.Based on this,this paper systematically analyzed the mechanism of inflammatory reaction during one lung ventilation in recent two years,and elaborated the related prevention research of inflammatory reaction during one lung ventilation from the aspects of protective strategy,anesthesia mode or drug,pretreatment scheme,etc.,and to provide guidance for the patients who need to accept thoracotomy in the later clinical stage,so as to reduce the incidence rates of postoperative adverse reactions and complications.
