毓麟珠联合鹿胎膏治疗卵巢低反应性不孕症及对妊娠结局的影响

摘　　要：目的探讨毓麟珠与鹿胎膏联合应用在卵巢低反应(POR)状态下的促排卵疗效,拓宽POR治疗的临床思路,并为中医药在辅助生殖技术中的推广应用提供循证医学依据。方法86例卵巢低反应不孕患者随机分为2组,毓麟珠加减与鹿胎膏为治疗组(A组),黄体期补佳乐预处理为对照组(B组)。3个月经周期后,比较排卵率、子宫内膜厚度变化、妊娠率、流产率。结果A组排卵率、内膜厚度、A型内膜比例均高于B组,差异有统计学意义(P<0.05)。B组治疗后APC、E2、FSH、AMH、RI与治疗前比较,差异无统计学意义(P>0.05),A组治疗后E2、AMH水平高于治疗前,RI水平低于治疗前,差异有统计学意义(P<0.05)。B组治疗后E2、AMH水平低于A组,RI水平高于A组,差异均有统计学意义(P<0.05)。A组服药后月经量少、腰膝酸软、畏寒肢冷、烦躁易怒症状评分较前显著改善,且明显低于B组治疗后,差异均有统计学意义(P<0.05);治疗前后潮热面红、失眠多梦、神疲乏力、头晕耳鸣的症状评分差异无统计学意义(P>0.05)。B组治疗前后中医症状评分比较,差异无统计学意义(P>0.05)。治疗后A组妊娠率高于B组,流产率低于B组,但差异无统计学意义(P>0.05)。结论毓麟珠加减与鹿胎膏联合治疗卵巢低反应不孕,可调整卵巢对促性腺激素的反应,改善子宫内膜的容受性,提高临床妊娠率,临床疗效满意。Objective To investigate the clinical efficacy of Yulinzhu combined with deer placenta cream in treatment of poor ovarian responses(POR)infertility and its effects on pregnancy outcome,so as to provide evidence-based medicine basis for the promotion and application of TCM in assisted reproductive technology.Methods A total of 86 patients with POR infertility were randomly divided into two groups.The patients in group A were treated by Yulinzhu combined with deer placenta cream,however,the patients in group B were treated by progynova at luteal phase.After 3 menstrual cycles,the ovulation rate,endometrial thickness change,pregnancy rate and abortion rate were observed and compared between the two groups.Results The ovulation rate,intima thickness and type A intima ratio in group A were significantly higher than those in group B(P<0.05).There were no significant differences in APC,E2,FSH,AMH,RI in group B before treatment and after treatment(P>0.05).After treatment the levels of E2 and AMH in group A were significantly increased,but RI levels were significantly decreased(P<0.05).After treatment,the levels E2 and AMH in group B were significantly lower than those in group A,however,the RI levels were significantly higher than those in group A(P<0.05).After treatment the scores of menstrual flow,low back and knees,cold limbs,irritability and irritability in group A were significantly improved,moreover,which were significantly lower than those in group B(P<0.05).There were no significant differences in the symptom scores of hot flashes,flushing,insomnia,fatigue,dizziness and tinnitus before treatment and after treatment(P>0.05).There were no significant differences in TCM symptom scores in group B before treatment and after treatment(P>0.05).After treatment,the pregnancy rate in group A was higher than that in group B,and the abortion rate was lower than that in group B,but the differences were not statistically significant(P>0.05).Conclusion The combination application of Yulinzhu combined with deer placenta cream in
