摘　　要：目的观察射频超声检测颈动脉弹性对闽东畲族无症状高血压的诊断价值。方法选取212例闽东地区畲族人群无症状高血压患者(病例组),另以同期该地区同族220名血压正常健康者为对照组。采用射频超声检测2组颈动脉内-中膜厚度(IMT)、扩张系数(DC)、顺应性系数(CC)、硬度指数(α)、血管弹性系数(β)、脉搏波速度(PWVβ),对比组间各参数差异;采用受试者工作特征(ROC)曲线分析上述参数对无症状高血压的诊断效能。结果病例组IMT、α、β及PWVβ均高于对照组,DC、CC均低于对照组(P均<0.01)。ROC曲线结果显示,IMT、DC、CC、α、β及PWVβ单独诊断无症状高血压的AUC为0.637~0.764,敏感度74.6%~88.9%,特异度60.8%~74.8%;各参数联合诊断的AUC为0.925,敏感度为93.2%,特异度84.6%。结论闽东畲族无症状高血压患者颈动脉IMT增加、僵硬度增高;射频超声多参数联合分析可早期检出无症状高血压。Objective To explore the diagnostic value of carotid artery elasticity measured with radiofrequency ultrasound for diagnosis of asymptomatic hypertension in Mindong She minority.Methods Totally 212 asymptomatic hypertensive patients from Mindong She minority were enrolled as case group,while 220 healthy people with normal blood pressure of the same ethnic group and region were taken as control group.Radiofrequency ultrasound was used to detect intra-media thickness(IMT)and dilatation coefficient(DC),compliance coefficient(CC),stiffness index(α),vascular elasticity coefficient(β)and pulse wave velocity(PWVβ)of carotid artery in 2 groups,and then the parameters were compared between groups.The receiver operating characteristic(ROC)curve was used to analyze the diagnostic efficacy of the above parameters for asymptomatic hypertension.Results IMT,α,βand PWVβof carotid artery in case group all increased,whereas DC and CC decreased in control group compared with those in control group(all P<0.01).ROC curve results showed that the area under the curve(AUC)of IMT,DC,α,β,PWVβ,DC and CC alone for diagnosis of asymptomatic hypertension was 0.637 to 0.764,the sensitivity ranged from 74.6%to 88.9%,and the specificity ranged from 60.8%to 74.8%.AUC of diagnosis of asymptomatic hypertension with the combination of the above parameter was 0.925,with sensitivity of 93.2%and specificity of 84.6%.Conclusion IMT and stiffness of carotid artery increased in asymptomatic hypertension patients of Mindong She minority.Radiofrequency ultrasound multi-parameter analysis could early detect asymptomatic hypertension.
