逆行髓内钉、加压钢板内固定治疗股骨中下段骨折的效果对比研究

摘　　要：目的就逆行髓内钉、加压钢板内固定治疗股骨中下段骨折的效果进行对比研究分析。方法选取我院2014年1月—2020年12月收治的股骨中下段骨折70例为研究对象,根据内固定方式的不同,分为观察组40例(逆行髓内钉治疗)和对照组30例(加压钢板内固定治疗)。比较2组手术切口大小、手术出血量、手术和住院时间、临床指标、膝关节功能恢复情况及术后并发症发生情况。结果与对照组比较,观察组手术切口更小、手术时间和住院时间更短、手术出血量更少(P<0.05)。观察组骨折愈合时间、末次随访股骨角与股骨角变化量低于对照组(P<0.01)。观察组膝关节功能恢复优良率高于对照组,术后并发症发生率低于对照组(P<0.05)。结论逆行髓内钉较加压钢板内固定治疗股骨中下段骨折的疗效更优且稳定性更强,术后并发症少,更利于骨折的愈合和膝关节功能的恢复。Objective To compare between retrograde intramedullary nailing and internal fixation of compression plate in treatment of patients with fracture of the middle and lower femur.Methods A total of 70 patients with fracture of the middle and lower femur admitted between January 2014 and December 2020 were selected as the research subjects,and the patients were divided into observation group(n=40,retrograde intramedullary nailing treatment)and control group(n=30,internal fixation of compression plate treatment)according to different methods of internal fixation.Size of surgical incision,intraoperative volume of blood loss,operative duration,length of hospitalization,clinical indicators,recovery of knee joint function and postoperative incidence rate of complications were compared in two groups.Results Compared with those in control group,observation group had shorter surgical incisions,operative duration and length of hospitalization,and less intraoperative volume of blood loss(P<0.05).In observation group,fracture healing time,the femoral angle at the last follow-up and change in femoral angle were shorter or smaller than those in control group(P<0.01);excellent and good rate of recovery of knee joint function was significantly higher than that in control group,while incidence rate of complications was significantly lower than that in control group(P<0.05).Conclusion Retrograde intramedullary nailing has better curative effect and stronger stability in treatment of patients with fracture of the middle and lower femur compared with those by internal fixation of compression plate.It has fewer postoperative complications and is more conducive to fracture healing and recovery of knee joint.
