摘  要:目的探讨个性化聚醚醚酮(PEEK)植入物对儿童颅骨缺损修补的临床治疗效果。方法选取湖南省儿童医院神经外科自2017年3月到2020年4月收治的去骨瓣减压术后行颅骨修补术的9例颅骨缺损患儿(2~10岁)为研究对象。利用三维重建技术和三维打印技术制备个性化聚醚醚酮植入物,分别对9例患儿行颅骨缺损修补术治疗。采用回顾性分析法,比较术前术后的影像学图片,评价颅骨缺损的修复情况。结果所有患儿PEEK植入物贴合完好,颅骨修补塑性良好。CT与MRI检查未发现伪影干扰,患儿术后均未出现感染、颅内出血、皮下积液等并发症,未出现中枢神经系统以及全身其他器官的新发功能障碍。随访时间均满半年以上,患儿头颅外型发育正常,切口愈合良好,各项神经功能及认知功能恢复明显。结论对于儿童颅骨缺损修补手术,采用个性化PEEK植入物可更好地匹配缺损伤口形状,对脑组织起到保护作用,且PEEK材料的组织相容性良好,术后无不良反应。3D打印个性化PEEK植入物技术可广泛应用于儿童颅骨缺损修补术中。Objective To investigate the clinical therapeutic effect of personalized polyetheretherketone(PEEK)implant on repairing children’s skull defects.Methods Nine cases of pediatric patients with skull defects(2-10 years)who underwent skull repair after debone valve decompression in Neurosurgery Department of Hunan Chirdren’s Hospital from March 2017 to April 2020 were selected.Nine children were treated with skull defect repair respectively with personalized PEEK implant which was prepared through three-dimensional reconstruction technology and three-dimensional printing technology.Retrospective analysis was applied to compare the preoperative and postoperative imaging pictures to evaluate the repair of skull defects.Results All children with PEEK implants were well fitted and well plastic for skull repair.CT and MRI examination showed no artifacts interference and the PEEK implant was stably fitted in pediatric patients after operation.All patients had no infection,intracranial hemorrhage,and subcutaneous effusion as well as dysfunction of organs of the body including central nervous system,and no other long-term complications appeared.All patients were followed up for more than half a year.The incision recovered well with no deformity in the head of the child.Neurological and cognitive functions recovered significantly.Conclusion The application of personalized PEEK implants can better match the shape of the defect and protect the brain tissue.The PEEK material has good biocompatibility and no adverse reactions occurred after surgery.3D printing personalized PEEK implant technology can be widely used in children’s cranioplasty.
