摘　　要：目的探讨超声引导下经外周中心静脉置管(PICC)的效果和安全性。方法选取2018年1-6月5家合作医院新生儿科重症监护室进行PICC置管的新生儿200例为研究对象,随机分为观察组和对照组,每组各100例。观察组患儿采用超声引导下进行PICC置管,对照组患儿采用盲穿法进行PICC置管。比较两组患儿置管留置时间、一次性置管成功率及总并发症发生率。结果观察组患儿一次性置管成功率高于对照组,穿刺至成功的时间低于对照组,差异均有统计学意义(均P<0.05)。观察组患者并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论通过超声实时引导下进行PICC置管能提高置管成功率,可有效解决传统盲穿法进行置管困难的问题,减少反复穿刺给患儿带来的痛苦,降低并发症发生率,为操作者置管提供了极大便利。同时可降低新生儿死亡率,提高救治成功率,减少并发症和后遗症的发生。Objective To investigate the efficacy and safety of ultrasound-guided Peripherally inserted central cathete(PICC).Methods A total of 200 neonates receiving PICC catheterization in neonatal intensive care units of 5 cooperative hospitals from January to June 2018 were selected as the research subjects and randomly divided into observation group and control group,with 100 cases in each group.The observation group used ultrasound-guided PICC catheterization,and the control group used blind puncture method for PICC catheterization.The indwelling time,the success rate of one-time catheterization and the incidence of total complications were compared between the two groups.Results The success rate of one-time catheterization in the observation group was higher than that in the control group,and the time from puncture to success was lower than that in the control group,the differences were statistically significant(P<0.05).The total incidence of complications in the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion PICC catheterization under the real-time guidance of ultrasound can improve the success rate of catheterization,effectively solve the difficulty of traditional blind puncture catheterization,reduce the pain of repeated puncture to children,reduce the incidence of complications,and provide great convenience for the operator catheterization.At the same time,it can reduce the neonatal mortality rate,improve the success rate of treatment,and reduce the occurrence of complications and sequelae.
