摘 要:目的探讨全血细胞分析指标与原发性肾病综合征(PNS)患者炎症反应和肾功能损害的相关性。方法收集绵阳市中心医院126例PNS患者(PNS组)及133名同期进行健康体检的正常人群(对照组)作为研究对象,采集静脉血进行血常规和肾功能指标分析。比较红细胞体积分布宽度变异系数(RDW-CV)、中性粒细胞/淋巴细胞比值(NLR)和血小板/淋巴细胞比值(PLR)在两组间的差异。根据慢性肾脏病(CKD)分期将不同程度肾功能损害的PNS患者分为3个亚组,比较不同亚组间各指标的差异。分析CKD分期与各指标的相关性。结果PNS组RDW-CV、NLR和PLR水平均较正常对照组升高,差异有统计学意义(Z=-2.203、-4.558、-2.042,P均<0.05)。PNS各亚组间比较显示RDW-CV、NLR和PLR差异有统计学意义(F=7.644、8.010、6.081,P均<0.05)。Spearman相关分析显示,CKD分期与RDW-CV和NLR呈正相关(r=0.242、0.211,P均<0.05),与PLR呈负相关(r=-0.219,P<0.05)。结论监测RDW-CV和NLR的变化对PNS患者的诊治有一定应用价值,可能成为评估炎症反应和肾功能损害的潜在指标。Objective To investigate the correlation between indexes of complete blood count and inflammation with impaired renal function in patients with primary nephrotic syndrome(PNS).Methods 126 patients with PNS(PNS group)and 133 healthy subjects(control group)who underwent healthy physical examination at the same time were enrolled to the study.Venous blood was collected for complete blood count and renal function indexs analysis.The differences of red blood cell distribution width-coefficient of variation(RDW-CV),neutrophil to lymphocyte ratio(NLR)and platelet to lymphocyte ratio(PLR)between the two groups were compared.According to the chronic kidney disease(CKD)stage,PNS patients with different degrees of renal impairment were divided into three subgroups,and the differences of indexes among subgroups were analyzed.The correlations between CKD stage and indexes were analyzed.Results RDW-CV,NLR and PLR of patients with PNS were all significantly higher than those in control subjects(Z=-2.203,-4.558,-2.042,all P<0.05).Statistical differences of RDW-CV,NLR and PLR were showed among PNS subgroups(F=7.644,8.010,6.081,all P<0.05).Spearman correlation analysis revealed that CKD stage was positively correlated with RDW-CV and NLR(r=0.242,0.211,P<0.05),and negatively correlated with PLR(r=-0.219,P<0.05).Conclusion Monitoring the changes of RDW-CV and NLR had potential value in assessment of inflammation and renal impairment for patients with PNS.
