摘  要:目的:通过实验观察针康法对慢性低灌注(CCH)大鼠学习记忆能力的影响,脑白质病变以及胼胝体内少突胶质细胞的变化,由此探讨该方法对于改善慢性低灌注所致认知障碍的机制。方法:取215~265 g雄性清洁级Wistar大鼠30只,按随机数字表法分为假手术组、模型组、针刺组、康复组及针康组,每组6只。采用双侧颈总动脉结扎法(2-VO)制备CCH模型。各组大鼠分别按照实验计划进行相应治疗。4周后,通过Morris水迷宫的定位航行和空间探索实验检测大鼠的行为学变化;HE染色检测大鼠脑白质变性程度;ELISA检测TNF-α、IL-1β水平;免疫组化法检测大鼠胼胝体部位少突胶质细胞(OLGs)标志物阳性数目的表达;激光多普勒血流仪(LDF)检测大鼠脑血流量的变化。结果:与模型组相比,各治疗组大鼠逃避潜伏期短缩,搜索原平台次数增加,脑白质神经纤维排列紧密,胼胝体空泡面积明显减少,TNF-α、IL-1β水平减少,OLGs的阳性表达数增加,大脑皮层血流量明显增加,且针康组的改善较为明显,差异具有统计学意义(P<0.05)。结论:针刺联合康复法能够显著改善慢性低灌注大鼠认知功能障碍后学习记忆能力,且疗效优于单纯的针刺或康复治疗,这可能与针康法能够进一步增加胼胝体内OLGs的表达有关。Objective:To observe the effects of acupuncture-rehabilitation therapy on learning and memory abilities of rats with chronic cerebral hypoperfusion(CCH)and its influence to the changes of oligodendrocytes(OLGs)in the callus,so as to explore the mechanism of acupuncture-rehabilitation therapy in improving learning and memory abilities due to CCH.Methods:30 Wistar rats were randomly divided into the sham operation group,the model group,the acupuncture group,the rehabilitation group and the acupuncture-rehabilitation group,with 6 rats in each group.The CCH model was prepared by bilateral vessel occlusion(2-VO).The rats in each group were intervened according to the experimental plan.After 4 weeks,the rats’behavior was measured through positioning navigation and space exploration experiments in the Morris water maze.The degeneration degree of alba was detected by HE staining.The levels of TNF-αand IL-1βwere measured by ELISA method.Immunohistochemical method was used to detect the positive expression of OLGs in the corpus callosum.The changes of cerebral blood flow were assessed by laser Doppler flowmeter.Results:Compared to those in the model group,the incubation period was shortened,the original platform search was increased,the nerve fibers of alba were tightly arranged,the cavitation area of the corpus callosum was significantly reduced,the levels of TNF-αand IL-1βwere significantly decreased,the positive expression of OLGs was significantly increased,and the blood flow of the cerebral cortex was also significantly increased in the acupuncture-rehabilitation group,the rehabilitation group and the acupuncture group(P<0.05);of which the improvements were more significant in the acupuncture-rehabilitation group than the other two treatment groups(P<0.05).Conclusion:The combination of acupuncture and rehabilitation can significantly improve the learning and memory abilities in the rats with cognitive dysfunction due to CCH,and the curative effect is better than acupuncture or rehabilitation therapy alone,which
