摘　　要：目的探讨手法复位夹板联合石膏外固定治疗青少年胫骨远端三平面骨折的效果。方法回顾性分析2015年1月—2018年12月广州市正骨医院收治的58例青少年胫骨远端三平面骨折患者的临床资料,根据治疗方式的不同将其分为治疗组与对照组,每组各29例。治疗组患者采用手法复位夹板联合石膏外固定方法,对照组采用切开复位内固定方法。比较两组患者的临床疗效、不良反应及骨折愈合时间。结果全部病例均获得随访,时间6~18个月,平均11个月。两组患者的治疗优良率比较,差异无统计学意义(P>0.05)。治疗组患者的不良反应发生率低于对照组,差异有统计学意义(P<0.05)。治疗组患者的骨折愈合时间短于对照组,差异有统计学意义(P<0.05)。结论采用手法复位夹板联合石膏外固定治疗胫骨远端三平面骨折的临床效果显著,值得临床推广应用。Objective To explore the effect of manual reduction splint combined with plaster external fixation in the treatment of triplane fracture of distal tibia in adolescents.Methods The clinical data of 58 adolescent patients with triplane fracture of the distal tibia who were admitted to Guangzhou Bone Setting Hospital from January 2015 to December 2018 were retrospectively analyzed,and they were divided into the treatment group and the control group according to different treatment method,with 29 cases in each group.The treatment group was treated with manual reduction splint combined with plaster external fixation,while the control group was treated with open reduction and internal fixation.Clinical efficacy,adverse reactions and fracture healing time were compared between the two groups.Results All patients were followed up for 6 to 18 months,with an average of 11 months.There was no statistically significant difference in the excellent and good rate of treatment between the two groups(P<0.05).The incidence of adverse reactions in the treatment group was lower than that in the control group,and the difference was statistically significant(P<0.05).The fracture healing time in the treatment group was shorter than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Manual reduction splint combined with plaster external fixation in the treatment of triplane fracture of distal tibia has significant clinical effect and is worthy of clinical promotion and application.
