摘　　要：目的探讨针刺辅助麻醉对腹腔镜胆囊切除术患者疼痛程度及血流动力学的影响。方法选取2019年1月—2020年1月于辽宁中医药大学附属医院行腹腔镜胆囊切除术的72例患者作为研究对象,按照随机数字表法分为对照组(36例)与试验组(36例)。对照组予以全身麻醉,试验组予以针刺辅助全身麻醉。比较两组手术情况,术前、术后12、24 h的疼痛程度及麻醉前(T0)、气管插管时(T1)、气腹建立后15 min(T2)、麻醉结束时(T3)的血流动力学。结果两组术中出血量、气腹时间、麻醉时间比较,差异无统计学意义(P>0.05)。两组术后12、24 h的视觉模拟量表(VAS)评分均低于术前,且试验组术后12、24 h的VAS评分低于对照组,差异有统计学意义(P<0.05)。试验组T1~T3的心率(HR)、平均动脉血压(MAP)均低于对照组,差异有统计学意义(P<0.05);而两组各时间点血氧饱和度(SpO2)比较,差异无统计学意义(P>0.05)。结论针刺辅助麻醉可改善腹腔镜胆囊切除术患者血流动力学,减轻患者疼痛程度,且对患者手术过程无明显影响。Objective To investigate the effect of acupuncture assisted anesthesia on pain and hemodynamics in patients undergoing laparoscopic cholecystectomy.Methods A total of 72 patients who underwent laparoscopic cholecystectomy in Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from January 2019 to January 2020 were selected as the research subjects,and were divided into control group(36 cases)and experimental group(36 cases)according to random number table method.The control group was given general anesthesia,and the experimental group was given acupuncture as auxiliary general anesthesia.The surgical conditions,the pain degree before and 12 and 24 h after surgery,the hemodynamics before anesthesia(T0),endotracheal intubation(T1),15 min after pneumoperitoneum establishment(T2),and at the end of anesthesia(T3)of the two groups were compared.Results There were no significant differences in the intraoperative blood loss,pneumoperitoneum time and anesthesia time between the two groups(P>0.05).The visual analogue scale(VAS)scores at 12 and 24 h after surgery in both groups were lower than those before surgery,and the VAS scores in the experimental group were lower than those in the control group at 12 and 24 h after surgery,the differences were statistically significant(P<0.05).Heart rate(HR)and mean arterial blood pressure(MAP)in experimental groups at T1-T3 were lower than those in control group,the differences were statistically significant(P<0.05).There was no significant difference in blood oxygen saturation(SpO2)between the two groups at each time point(P>0.05).Conclusion Acupuncture assisted anesthesia can improve hemodynamics and alleviate pain in patients undergoing laparoscopic cholecystectomy,and has no side effect on the operation process.
