摘　　要：目的评价外周超声支气管镜(EBUS-GS)联合虚拟支气管镜(VBN)引导下经肺活检对老年患者肺部结节(PPLs)的诊断价值。方法对43例经过胸部CT或者PET/CT明确的PPLs且年龄≥65岁患者进行VBN联合EBUS-GS肺活检以及毛刷检查。结果43例中,28例确诊肺癌(腺癌18例,鳞癌7例,小细胞癌3例),4例为特殊感染病变(结核2例,曲霉菌1例,隐球菌1例),假阴性3例。诊断的敏感度为89.3%,特异度为100%。发生气胸1例。结论EBUS-GS联合VBN是诊断老年患者外部肺部结节性质有效的方法。Objective To evaluate the value of translung biopsy guided by peripheral ultrasound bronchoscopy combined with virtual bronchoscopic navigation in the diagnosis of peripheral pulmonary nodules in elderly patients.Methods 43 patients with peripheral pulmonary nodules confirmed by chest CT or PET/CT and age≥65 years old were examined by translung biopsy guided by peripheral ultrasound bronchoscopy combined with virtual bronchoscopic navigation.Results Among the 43 cases,28 cases were diagnosed with lung cancer(18 cases of adenocarcinoma,7 ones of squamous cell carcinoma and3 ones of small cell carcinoma),4 ones were special infectious lesions(2 ones of tuberculosis,1 one of Aspergillus and 1 one of Cryptococcus),and 3 ones were false negative.The sensitivity of diagnosis was 89.3%,and the specificity was 100%.Pneumothorax occurred in 1 case.Conclusion Translung biopsy guided by peripheral ultrasound bronchoscopy combined with virtual bronchoscopic navigation is an effective and safe method for the diagnosis of peripheral pulmonary nodules in the elderly.
