显微镜下精索静脉结扎术治疗原发性精索静脉曲张患者的效果

摘　　要：目的:观察显微镜下精索静脉结扎术治疗原发性精索静脉曲张患者的效果。方法:选取70例精索静脉曲张患者为研究对象,以随机数字表法分为研究组和对照组各35例。对照组接受传统开放结扎术治疗,研究组接受显微镜下精索静脉结扎术治疗,比较两组手术指标水平、并发症发生率、复发率和精液质量指标水平。结果:研究组手术用时和住院时长均短于对照组,术后24 h视觉模拟评分法评分低于对照组,差异有统计学意义(P<0.05);研究组并发症发生率为5.71%(2/35),低于对照组的25.71%(9/35),差异有统计学意义(P<0.05);研究组复发率为2.86%(1/35),低于对照组的22.86%(8/35),差异有统计学意义(P<0.05);治疗后,研究组精子活率高于对照组,差异有统计学意义(P<0.05);两组精子密度和a+b级精子比例比较,差异均无统计学意义(P>0.05)。结论:显微镜下精索静脉结扎术治疗原发性精索静脉曲张患者可缩短手术用时和住院时长,降低疼痛评分、并发症发生率和复发率,提高精子活率,效果优于传统开放结扎术治疗。Objective:To effects of microscopic ligation of spermatic vein in treatment of patients with primary varicocele.Methods:70 patients with varicocele were selected as the research objects,and were divided into study group and control group by using the random number table,35 cases in each group.The control group received traditional open ligation,while the study group was given the microscopic ligation of spermatic vein.The surgical index levels,the complication rate,the recurrence rate and the semen quality index levels were compared between the two groups.Results:The operation time and hospitalization time in the study group were shorter than those in the control group,the visual analog pain score 24 h after the surgery was lower than that in the control group,and the differences were statistically significant(P<0.05).The complication rate in the study group was 5.71%(2/35),which was lower than 25.71%(9/35)in the control group,and the difference was statistically significant(P<0.05).The recurrence rate in the study group was 2.86%(1/35),which was lower than 22.86%(8/35)in the control group,and the difference was statistically significant(P<0.05).After the treatment,the sperm motility rate of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).However,there were no statistically significant differences in the sperm density and the a+b grade sperm between the two groups(P>0.05).Conclusions:Microscopic ligation of spermatic vein in the treatment of the patients with primary varicocele can shorten the operation time and the hospitalization time,reduce the pain score,the complication rate and the recurrence rate,and improve the sperm motility rate.Moreover,it is superior to traditional open ligation treatment.
