摘　　要：目的:探究临床药师参与围术期血糖管理的模式及成效。方法:选取2019年9月至12月参与围术期血糖管理与内分泌科会诊的高血糖患者,分为干预组与对照组。对照组仅作普通会诊。干预组由医生、药师、护士根据围术期高血糖管理流程进行管理;临床药师每日到患者床边查房,提供用药教育、咨询服务,记录患者饮食、用药、血糖及药物不良反应发生情况。管理结束分析干预前后血糖参数、达标情况、住院时间等。结果与结论:干预组血糖参数及达标情况明显优于对照组,住院时间明显少于对照组。临床药师参与围术期血糖管理模式为其它慢病管理提供参考。Objective:To explore the mode and effect of clinical pharmacists participating in perioperative blood glucose management.Methods:From September 2019 to December 2019,perioperative hyperglycemia patients in the endocrinology department were selected to participate in perioperative blood glucose management or general consultation.The perioperative patients participating in blood glucose management(the management group)were managed according to the perioperative hyperglycemia management process.Clinical pharmacists went to the patient’s bedside for rounds every day to provide medication education and consulting services,and recorded the patient’s diet,medication,blood sugar and adverse drug reactions.The blood glucose parameters,compliance status,hospital stay and other data before and after the management were analyzed.Results:The results of the perioperative blood glucose management group with clinical pharmacists were significantly better than those of the control group,and the hospital stay was significantly shorter in the management group.The management model can provide a reference for future clinical pharmacists to participate in other chronic disease management.
