正压通气联合布地奈德雾化治疗COPD合并呼吸衰竭的疗效观察

摘　　要：目的分析慢性阻塞性肺疾病(COPD)合并呼吸衰竭疾病特点,评价正压通气+布地奈德雾化治疗合并症的预后效果。方法选择我院2019年3月至2020年2月诊治的COPD合并呼吸衰竭疾病患者,经伦理委员会审核批准,总计80例。对症支持治疗基础上采取1∶1比例法进行分组治疗,对照组合并症患者采取正压通气治疗,观察组合并症患者采取正压通气联合布地奈德雾化治疗,每组40例。结果组间治疗效果比较,观察组合并症患者治疗总有效率(95.00%)高于对照组(70.00%),P<0.05;组间治疗后动脉血气以及肺功能指标水平比较治疗前改善,观察组合并症患者改善幅度大于对照组,P<0.05。结论正压通气联合布地奈德雾化治疗的基础上可以促进COPD合并呼吸衰竭患者动脉血气、肺功能指标改善,大大提升了临床治疗效果。Objective To analyze the characteristics of COPD(chronic obstructive pulmonary disease)combined with respiratory failure,and to evaluate the prognostic effect of positive pressure ventilation+budesonide atomization in the treatment of complications.Methods A total of 80 patients with COPD and respiratory failure who were diagnosed and treated in our hospital from March 2019 to February 2020 were reviewed and approved by the ethics committee.On the basis of symptomatic and supportive treatment,a 1∶1 ratio method was adopted for group treatment.The control group patients with comorbidities were treated with positive pressure ventilation,and the observation group combination patients with comorbidities were treated with positive pressure ventilation combined with budesonide atomization.There were 40 cases in each of the two groups.Results Comparing the treatment effects between the groups,the total effective rate(95.00%)of patients with comorbidities in the observation combination was higher than that of the control group(70.00%),P<0.05;the arterial blood gas and lung function index levels after treatment between the groups were improved compared with before treatment.The improvement of patients with comorbidities in the observation combination was greater than that in the control group,P<0.05.Conclusion Based on positive pressure ventilation combined with budesonide nebulization therapy,it can promote the improvement of arterial blood gas and pulmonary function in patients with COPD and respiratory failure,and greatly enhance the clinical treatment effect.
