以行为分阶段改变理论为指导的护理干预对低位直肠癌手术患者术后排便功能及生活质量的影响

摘　　要：目的探讨以行为分阶段改变理论为指导的护理干预对低位直肠癌手术患者应用效果。方法选取2017年1月至2019年12月我院收治的72例低位直肠癌保肛手术患者,按随机数字表法分为两组,各36例。对照组给予常规护理,观察组采用以行为分阶段改变理论为指导的护理干预。比较两组排便功能、肛门失禁程度及生活质量。结果观察组干预6个月后排便功能优于对照组,肛门失禁程度轻于对照组,差异有统计学意义(P<0.05);观察组干预6个月后生活方式(3.20±0.16)分、抑郁/自我感受(3.31±0.11)分、心理应付/行为(2.97±0.46)分、窘迫尴尬(3.28±0.27)分,高于对照组,差异有统计学意义(P<0.05)。结论以行为分阶段改变理论为指导的护理干预能够促进低位直肠癌手术患者术后排便功能恢复,减轻肛门失禁程度,提升生活质量。Objective To explore the effect of nursing intervention guided by the theory of behavioral change in stages on patients undergoing surgery for low rectal cancer.Methods A total of 72 patients with low rectal cancer undergoing anus preservation surgery in our hospital from January 2017 to December 2019 were selected and divided into two groups according to a random number table,with 36 cases in each group.The control group was given routine nursing care,and the observation group was treated with nursing intervention guided by the theory of behavioral change in stages.The defecation function,the degree of anal incontinence and the quality of life were compared between the two groups.Results The defecation function of the observation group was better than that of the control group after 6 months of intervention,and the degree of anal incontinence was lighter than that of the control group,the difference was statistically significant(P<0.05);the observation group's lifestyle(3.20±0.16)points after 6 months of intervention depression/selffeeling(3.31±0.11)score,psychological coping/behavior(2.97±0.46)score,distress and embarrassment(3.28±0.27)score,higher than the control group,the difference was statistically significant(P<0.05).Conclusion Nursing intervention guided by the theory of behavioral changes in stages can promote the recovery of bowel function in patients with low rectal cancer after surgery,reduce the degree of anal incontinence,and improve the quality of life.
