摘　　要：目的探讨羊水最大深度差值对单绒毛膜双羊膜囊(MCDA)双胎妊娠发生双胎输血综合征(TTTS)的早期预测价值。方法根据入排标准,将最终诊断为合并TTTS的38例MCDA双胎作为病例组,选取同期无并发症的174例MCDA双胎作为对照组进行回顾性分析,采用ROC曲线分析孕早期和孕中期MCDA双胎间羊水最大深度差值分别对TTTS的预测价值。结果孕早期羊水最大深度差值对MCDA双胎妊娠孕妇发生TTTS的预测无统计学意义(P=0.918)。而孕中期羊水最大深度差值明显大于对照组(P=0.001),孕中期羊水最大深度差值≥21.5 mm时,预测TTTS发生的特异性为96.6%,敏感度为89.5%,阳性预测值为85.0%,阴性预测值为97.7%。结论孕早期羊水最大深度差值对MCDA双胎妊娠发生TTTS无明显预测价值,孕中期产妇羊水最大深度差值对TTTS的发生有预测价值。Objective To explore the value of the difference of the maximal depth of amniotic fluid in the early prediction of twin-to-twin transfusion syndrome(TTTS)with monochorionic double amniotic sac(MCDA)twin pregnancy.Methods According to the entry criteria,38 MCDA twins with TTTS were selected as the case group and 174 MCDA twins without complications were selected as the control group,the ROC curve was used to analyze the predictive value of the difference of the maximal amniotic fluid depth between MCDA twins in the first and second trimester of pregnancy for TTTS.Results The difference of the maximal amniotic fluid depth in early pregnancy had no statistical significance in predicting the occurrence of TTTS in MCDA twin pregnancy(P=0.918).The difference of the maximal depth of amniotic fluid in the second trimester was significantly greater than that in the control group(P=0.001).When the difference of the maximal depth of amniotic fluid in the second trimester≥21.5 mm,the specificity,sensitivity and positive predictive value of TTTS were 96.6%,89.5%and 85.0%,respectively,the negative predictive value was 97.7.Conclusion The difference of maximum depth of amniotic fluid in early pregnancy has no significant value in predicting TTTS in MCDA twin pregnancy,and the difference of maximum depth of amniotic fluid in middle pregnancy has significant value in predicting TTTS.
