基于藏象学说的疲劳客观指标与量表的模型建立及分析

摘　　要：目的:基于现代生物物理指标与中医藏象学说结合,探究一套可实现疲劳早期检测及诊断的中西医结合方法和模型。方法:185例精神疲劳志愿者,采用多元线性回归分析方法,探究自主研发的“人体疲劳检测与评估系统”采集的皮电(SCL)、心率变异性(HRV)及血氧饱和度(SPO_(2))与精神疲劳分值的关系,构建一套生理指标与主观量表的疲劳诊断和评价模型。结果:多元线性回归分析结果为F=4.63,(P<0.05),说明该回归模型有意义,客观生理指标与主观量表之间存在线性关系。SCLmax、SCLmin、HRVhf、SPO_(2)mean均P<0.05。SCLmax、HRVhf、SCLmin的回归系数分别为-1.07、-1.31、-0.83,均会对精神疲劳分数产生负向影响;SPO_(2)mean回归系数为1.83,对精神疲劳分数产生正向影响。结论:现代生物物理指标与中医藏象学说相结合为精神疲劳构建一套中西医结合的检测模型,建立“藏象-生理指标-疲劳评价”的疲劳监控、评价模式,为中医学对疲劳诊断的客观化研究提供一个新的方向,同时也为疲劳的早期诊断及监测提供可靠有效的检测方法。Objective:To explore a set of integrative medicine methods and models that can realize early detection and diagnosis of fatigue,based on the combination of modern biophysical indicators and the theory of visceral manifestation in TCM.Methods:The relationship between electrical skin(SCL),heart rate variability(HRV)and blood oxygen saturation(SPO_(2)),which collected by the self-developed'Human Fatigue Detection and Evaluation System',and mental fatigue scores was explored in 185 mental fatigue volunteers by using the multiple linear regression analysis method,thus to construct a set of fatigue diagnosis and evaluation model of objective physiological indicators and subjective scales.Results:The results of multiple linear regression analysis were as follows:F=4.63,P<0.05,indicating that the regression model was statistically significant,and there was a linear relationship between objective physiological indicators and subjective scales.SCLmax,SCLmin,HRVhf and SPO_(2)mean were all P<0.05.The regression coefficients of SCLmax,HRVhf and SCLmin were-1.07,-1.31 and-0.83 respectively,indicating that SCLmax,SCLmin and HRVhf had a negative impact on mental fatigue scores.The regression coefficient of SPO_(2)mean was 1.83,which had a positive impact on mental fatigue scores.Conclusion:The combination of modern biophysical indicators and theory of visceral manifestation in TCM builds a set of detection models for mental fatigue.Establishing a fatigue monitoring and evaluation mode of'theory of visceral manifestationphysiological index-fatigue'evaluation provides a new direction for the objective study of fatigue diagnosis in TCM,and provides reliable and effective detection methods for the early diagnosis and monitoring of fatigue.
