浅析慢性阻塞性肺疾病急性加重患者序贯应用呼吸机家庭无创通气研究

摘　　要：慢性阻塞性肺疾病是一种由于长期暴露于有毒颗粒和气体所导致的气道和肺泡异常的疾病,其特征是持续存在的呼吸系统症状和气流受限,也称之为慢阻肺,是一种可以预防和治疗的疾病。慢阻肺疾病常由慢性支气管炎、肺气肿、支气管扩张等疾病发展而来,随着病程的增加患者临床症状会逐渐加重,且这种肺部损伤不可逆,极易引发肺部感染、心力衰竭、肺血栓栓塞等并发症。无创通气是近年来临床治疗慢阻肺的重要方式之一,经过长期的临床应用研究已经被推荐为慢阻肺的首选常规治疗手段,可以取得良好的治疗效果。但家庭无创通气治疗如何做好通气模式和通气时间等关键指标的控制尤为重要。本文先对呼吸机家庭无创通气在慢阻肺患者治疗中的意义作一简述,进而重点从呼吸机家庭无创通气治疗要点与管理要点两方面对慢性阻塞性肺疾病急性加重患者序贯应用呼吸机家庭无创通气作系统的分析综述,最后谈一谈对家庭无创通气在慢阻肺患者治疗中的发展趋势。Chronic obstructive pulmonary disease(COPD)is a preventable and treatable disease characterized by persistent respiratory symptoms and restricted airflow caused by prolonged exposure to toxic particles and gases.Copd often develops from chronic bronchitis,emphysema,bronchiectasis and other diseases.With the increase of the course of the disease,the clinical symptoms of patients will gradually worsen,and this kind of lung injury is irreversible,which can easily lead to pulmonary infection,heart failure,pulmonary thromboembolism and other complications.Noninvasive ventilation is one of the important clinical treatment methods for copd in recent years.After long-term clinical application studies,it has been recommended as the preferred routine treatment for COPD,which can achieve good therapeutic effects.However,it is particularly important to control the key indicators such as ventilation mode and ventilation duration in the treatment of noninvasive ventilation.In this paper,the significance of ventilator family noninvasive ventilation in the treatment of copd patients was briefly described,and then the main points of ventilator family noninvasive ventilation in the treatment and management of patients with acute exacerbations of chronic obstructive pulmonary disease were systematically analyzed and reviewed.
