摘　　要：目的探讨烧伤后自体岛状皮瓣移植存活情况及其影响因素。方法回顾性分析2017年1月至2020年1月在郑州大学附属洛阳中心医院实施自体岛状皮瓣移植的156例烧伤患者的临床资料,定期进行检查,统计皮瓣存活情况。收集所有患者的基线资料,经单因素与多因素分析影响烧伤后自体岛状皮瓣移植存活的影响因素。结果156例烧伤患者中自体岛状皮瓣存活126例,占比80.77%;自体岛状皮瓣失活30例,占比19.23%。经Logistic回归分析显示,术后使用地塞米松情况、术中操作不当情况、术后制动情况、术后创面保暖措施实施情况、术后渗血创面敷料更换是自体岛状皮瓣移植后失活的影响因素(O^R>1,P<0.05)。结论术后未使用地塞米松、术中操作不当、术后制动不佳、术后未实施创面保暖措施、术后渗血创面敷料未及时更换是岛状皮瓣移植后失活的影响因素,可据此实施干预措施。【Objective】To investigate the survival and influencing factors of autologous island flap transplantation after burn.【Methods】The clinical data of 156 burn patients who underwent autologous island flap transplantation in our hospital from January 2017 to January 2020 were retrospectively analyzed,the examination was taken regularly,the survival of the flaps was counted.The baseline data of all patients were collected,the influencing factors of the survival of autologous island flap transplantation after burn were analyzed by single factor and multiple factors.【Results】Among 156 burn patients,126 cases were survived with autologous island flap,accounting for 80.77%;30 cases were inactivated with autologous island flaps,accounting for 19.23%.Logistic regression analysis showed that postoperative use of dexamethasone,improper operation during operation,postoperative immobilization,implementation of postoperative wound warmth preservation measures,and postoperative bleeding wound dressing replacement were the influencing factors for the inactivation after autologous island flap transplantation(O^R>1,P<0.05).【Conclusion】No use of dexamethasone,improper operation during operation,poor immobilization after operation,the lack of wound warmth measures after operation,and the failure in replacing dressing for bleeding wound after operation are the influencing factors for the inactivation of island flap after transplantation,interventions can be implemented according to this.
