摘  要:目的:比较应用均整模式(FF)和无均整模式(FFF)下固定野调强(IMRT)在胸中段食管癌放疗中的剂量学差异。方法:选取2018年10月~2019年9月在本院就诊的20例胸中段食管癌患者分别进行FF-IMRT和FFFIMRT计划设计。比较两种计划的靶区和危及器官剂量(OAR)、均匀性指数(HI)、适形度指数(CI)、机器的跳数及加速器的出束时间。结果:FF-IMRT跳数低于FFF-IMRT[(684.4±98.7),(930.9±191.9)MU,P<0.05],FF-IMRT的出束时间大于FFF-IMRT[(69.48±11.64),(54.90±21.70)s,P<0.05]。两种计划的靶区剂量、均匀性、适形性及危及器官受量均未见明显差别。结论:两种计划均能满足临床治疗需求,FFF-IMRT相比于FF-IMRT具有更短的出束时间,提高了治疗效率。Objective:To compare the intensity modulated radiation therapy(IMRT)dosimetric differences between the radiotherapy of thoracic middle segment esophageal cancer using 6MV Xray flattening filter(FF)and flattening filter-flee(FFF)modes.Methods:20 patients with mid-thoracic esophageal cancer who were treated in our hospital from October 2018 to September 2019 were selected for FF-IMRT and FFFIMRT plan design.Compare the target area and organ-at-risk dose(OAR),uniformity index(HI),conformity index(CI),number of machine hops and the beam time of the accelerator of the two plans.Results:The hop count of FF-IMRT is lower than FFF-IMRT[(684.4±98.7),(930.9±191.9)MU,P<0.05],and the beam time of FF-IMRT is longer than FFF-IMRT[(69.48±11.64),(54.90±21.70)s,P<0.05].There was no significant difference in the target dose,uniformity,conformability and organ-at-risk dose of the two plans.Conclusion:The qualities of FF-IMRT and FFF-IMRT plans were comparable and both clinically acceptable.When comparing two plans,FFF-IMRT raises therapeutic efficiency by shortening the delivery time.
