系统性红斑狼疮病人血清人附睾分泌蛋白4和抗双链DNA抗体水平与病情活动性的相关性

摘　　要：目的探讨系统性红斑狼疮(SLE)病人血清人附睾分泌蛋白4(HE4)和抗双链DNA抗体(抗dsDNA抗体)水平与病情活动性的相关性。方法选取2017年1月至2018年12月在德阳市人民医院风湿免疫科进行诊治的74例SLE病人作为受试对象,另选取同期在该院进行体检的57例健康者作为对照组;按照SLE病情活动指数(SLEDAI)评分将74例SLE病人分为非活动期组35例(SLEDAI<10分)和活动期组39例(SLEDAI≥10分)。采用酶联免疫吸附测定(ELISA)双抗体夹心法检测血清HE4、抗dsDNA抗体水平。分析SLE病人血清HE4水平与抗dsDNA抗体及病情活动性的关系,并采用受试者工作特征曲线(ROC曲线)评估血清HE4、抗dsDNA抗体水平区分非活动期及活动期SLE病人的价值。结果SLE病人血清HE4、抗dsDNA抗体水平分别为(83.44±26.53)pmol/L、(1.53±0.49)μg/L,均明显高于对照组的(39.16±8.25)pmol/L、(0.42±0.13)μg/L(P<0.05)。SLE病人抗dsDNA抗体阳性血清HE4水平显著高于抗dsDNA抗体阴性者(P<0.05)。SLE活动期病人SLEDAI评分、血清HE4水平分别为(14.95±4.07)分、(95.76±29.55)pmol/L,均显著高于非活动期病人的(6.52±2.13)分、(60.34±18.12)pmol/L(P<0.05),且SLE病人血清HE4水平与抗dsDNA抗体水平、SLEDAI评分均呈正相关(P<0.05)。SLE病人血清HE4水平与补体(C3、C4)水平呈负相关(P<0.05),与免疫球蛋白(IgA、IgG、IgM)、抗核抗体(ANA)滴度及24 h尿蛋白定量均呈正相关(P<0.05)。ROC曲线分析表明血清HE4水平、抗dsDNA抗体水平均可区分非活动期及活动期SLE病人(P<0.05),且血清HE4水平效能优于血清抗dsDNA抗体水平。结论SLE病人血清HE4、抗dsDNA抗体水平升高与疾病活动程度呈正相关,二者水平升高有利于区分活动期及非活动期SLE病人。Objective To explore the correlation between serum level of human epididymal secretory protein 4(HE4)and anti-double-stranded DNA antibody(anti-dsDNA antibody)and disease activity in patients with systemic lupus erythematosus(SLE).Methods Seventy-four SLE patients who were diagnosed and treated in the Department of Rheumatology and Immunology of Deyang People's Hospital from January 2017 to December 2018 were selected as subjects,and another 57 healthy persons who had physical examination in the hospital during the same period were selected as the control group.According to SLE disease activity index(SLEDAI)score,the SLE patients were assigned into inactive group(35 cases with SLEDAI<10 points)and active group(39 cases with SLEDAI≥10 points).The levels of serum HE4 and anti-dsDNA antibody were detected by enzyme-linked immunosorbent assay(ELISA)double antibody sandwich method.The relationship between serum HE4 level and anti-dsDNA antibody and disease activity in SLE patients was analyzed,and the value of serum HE4 and anti-dsDNA antibody levels in differentiating inactive and active SLE patients was evaluated by receiver operating characteristic curve(ROC curve).Results The levels of serum HE4 and anti-dsDNA antibody in SLE patients were(83.44±26.53)pmol/L and(1.53±0.49)μg/L respectively,which were significantly higher than those in the control group[(39.16±8.25)pmol/L and(0.42±0.13)μg/L](P<0.05).The serum HE4 level of SLE patients with positive anti-dsDNA antibody was significantly higher than that of SLE patients with negative anti-dsDNA antibody(P<0.05).SLEDAI score and serum HE4 level in active SLE patients were(14.95±4.07)scores and(95.76±29.55)pmol/L respectively,which were significantly higher than those in inactive SLE patients[(6.52±2.13)scores and(60.34±18.12)pmol/L](P<0.05),and serum HE4 level was positively correlated with anti-dsDNA antibody level and SLEDAI score in SLE patients(P<0.05).Serum HE4 level in SLE patients was negatively correlated with complement(C3,C4)levels(P<0.05),and po
