摘 要:目的:评价高强度间歇有氧运动对冠心病病人心脏康复和生活质量的影响。方法:检索中国知网(CNKI)、万方数据库和维普数据库(VIP)等中文数据库以及Cochrane Library,PubMed,Web of Science,MEDLINE,CINAHI,Embase,JBI等英文数据库中有关高强度间歇有氧运动应用于冠心病病人中的随机对照试验,同时筛检纳入文章的参考文献。对符合纳入、排除标准的文献进行质量评价,采用RevMan 5.3软件进行Meta分析。结果:共纳入14篇文献,涉及698例病人。Meta分析结果显示,高强度间歇有氧运动在提高冠心病病人的峰值摄氧量(PeakVO 2)[SMD=0.55,95%CI(0.36,0.73),P<0.00001]、生活质量[SMD=0.34,95%CI(0.04,0.64),P=0.03]方面均优于对照组。但两组在静息心率(Resting HR)[SMD=0.14,95%CI(-0.05,0.34),P=0.16]、三酰甘油(TG)[SMD=-0.02,95%CI(-0.25,0.20),P=0.84]、收缩压(SBP)[SMD=1.06,95%CI(-0.74,2.87),P=0.25]、舒张压(DBP)[SMD=0.30,95%CI(-0.04,0.64),P=0.09]比较差异无统计学意义。结论:高强度间歇有氧运动与中等强度持续有氧运动相比能更有效提高冠心病病人的峰值摄氧量及生活质量,但两组在改善冠心病病人的静息心率、三酰甘油、收缩压、舒张压方面无明显差别。Objective:To evaluate the effect of high intensity intermittent aerobic exercise on the cardiac rehabilitation and quality of life(QOL)of patients with coronary heart disease(CHD).Methods:Randomized controlled trials(RCTs)related to the application of high-intensity intermittent aerobic exercise in CHD patients were retrieved from Chinese databases,including China National Knowledge Infrastructure(CNKI),Wanfang Database and VIP Database(VIP),and English databases including Cochrane Library,PubMed and Web of Science.The references of included articles were screened;the quality of the literature that meets the acceptance and exclusion standards was evaluated,and the RevMan 5.3 software was used for Meta-analysis.Results:A total of 14 articles were included,involving 698 patients.Meta-analysis results showed that high intensity intermittent aerobic exercise could improve the CHD patients′peak oxygen uptake(peakVO 2)[SMD=0.55,95%CI(0.36,0.73),P<0.00001]and QOL[SMD=0.34,95%CI(0.04,0.64),P=0.03],which was better than that in control group.The difference was not statistically significantin resting heart rate(RHR)[SMD=0.14,95%CI(-0.05,0.34),P=0.16],triglycerides[SMD=-0.02,95%CI(-0.25,0.20),P=0.84],systolic blood pressure(SBP)[SMD=1.06,95%CI(-0.74,2.87),P=0.25],diastolic blood pressure(DBP)[SMD=0.30,95%CI(-0.04,0.64),P=0.09]between the two groups.Conclusions:Compared with moderate-intensity continuous aerobic exercise,high-intensity intermittent aerobic exercise could improve the peakVO 2 and QOL of CHD patients more effectively.At the same time,there was no significant difference in RHR,triglycerides,SBP,and DBPbetween the two groups.
