摘 要:目的比较三维、四维超声筛查胎儿唇裂的临床价值,探讨四维超声筛查的最佳孕周。方法给予5000例高危妊娠孕妇三维、四维超声检查。以最终诊断结果为参照,比较三维、四维超声对胎儿唇裂的诊断效能。比较不同孕周胎儿的四维超声图像满意度及唇裂检出率。结果四维超声诊断胎儿唇裂的灵敏度、特异度、准确度、阳性预测值及阴性预测值均高于三维超声(P<0.05)。四维超声与最终诊断结果之间呈高度一致;三维超声与最终诊断结果之间呈中度一致。四维超声对Ⅰ度唇裂的诊断符合率高于三维超声,差异具有统计学意义(P<0.05)。孕22~25周胎儿的四维超声图像满意度最高;不同孕周四维超声对胎儿唇裂的检出率无显著差异(P>0.05)。结论四维超声对胎儿唇裂的诊断价值优于三维超声,可对胎儿唇裂予以灵敏、准确检出,以孕22~25周的四维超声图像质量最优,建议在孕22~25周采用四维超声对胎儿唇裂进行筛查。Objective To compare the clinical values of three-dimensional and four-dimensional ultrasound in screening fetal cleft lip,and to discuss the best gestational week of four-dimensional ultrasound screening.Methods A total of 5000 high-risk pregnant women were given three-dimensional and four-dimensional ultrasound examinations.The efficacy of three-dimensional and four-dimensional ultrasound in the diagnosis of fetal cleft lip was compared based on the final diagnostic results.The satisfaction of four-dimensional ultrasound images and the detection rate of cleft lip of fetuses at different gestational weeks were compared.Results The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of four-dimensional ultrasound in the diagnosis of fetal cleft lip were higher than those of three-dimensional ultrasound(P<0.05).There was a high consistency between four-dimensional ultrasound and the final diagnostic results;there was moderate consistency between three-dimensional ultrasound and the final diagnostic results.The diagnostic coincidence rate of four-dimensional ultrasound inⅠdegree cleft lip was higher than that of three-dimensional ultrasound,and the difference was statistically significant(P<0.05).The satisfaction of four-dimensional ultrasound image of fetus at 22-25 weeks of gestation was the highest;there was no significant difference in the detection rate of fetal cleft lip by four-dimensional ultrasound at different gestational weeks(P>0.05).Conclusion The four-dimensional ultrasound has better diagnostic value for fetal cleft lip compared with three-dimensional ultrasound,and it can detect fetal cleft lip sensitively and accurately.The image quality of four-dimensional ultrasound at 22-25 weeks of gestation is the best.It is suggested to screen fetal cleft lip by four-dimensional ultrasound at 22-25 weeks of gestation.
