糖皮质激素耳后注射治疗突发性聋的效果

摘　　要：目的比较耳后注射和全身应用糖皮质激素治疗突发性聋的效果。方法收集2018年1月至2020年12月阜阳市第六人民医院177例突发性聋患者病历资料,其中50例低频下降型突发性聋和43例中高频下降型突发性聋用甲泼尼龙琥珀酸钠行耳后注射、46例低频下降型突发性聋和38例中高频下降型突发性聋行静脉滴注地塞米松磷酸钠,比较两种治疗方法的效果。结果低频下降型突发性聋患者用甲泼尼龙琥珀酸钠耳后注射后有效率为88%,用地塞米松磷酸钠静脉滴注有效率为67.3%,差异有统计学意义(P<0.05);中高频下降型突发性聋用甲泼尼龙琥珀酸钠耳后注射有效率为76.7%,用地塞米松磷酸钠静脉滴注治疗有效率为60.5%,差异无统计学意义(P>0.05)。结论针对低频下降型突发性聋患者,耳后注射甲泼尼龙琥珀酸钠优于全身静脉滴注地塞米松磷酸钠;针对中高频下降型突发性聋患者,两种给药方法疗效差异无统计学意义。Objective To investigate the effect of retroauricular injection and systemic application of glucocorticoid in the treatment of sudden deafness.Method Collecting 177 cases of patients with sudden deafness treatment and curative effect of materials from January 2018 to December 2020 in the sixth people′s hospital of Fuyang City,including 50 cases of low frequency drop type of sudden deafness and frequency drop in 43 cases of sudden deafness with methyl prednisolone sodium succinate ears after injection,46 cases of low frequency drop type of sudden deafness and line type high frequency drop in 38 cases of sudden deafness intravenous drip dexamethasone sodium phosphate,compare two methods of treatment efficacy.Results The effective rate was 88.0%after retroaural injection of methylprednisolone sodium succinate and 67.3%after intravenous infusion of dexamethasone sodium phosphate,the difference was statistically significant(P<0.05).The effective rate of methylprednisolone sodium succinate by retroaural injection was 76.7%,and that of dexamethasone sodium phosphate by intravenous infusion was 60.5%,with no statistical significance(P>0.05).Conclusion For the patients with sudden deafness of low frequency decline,retroaural injection of methylprednisolone sodium succinate is better than systemic intravenous infusion of dexamethasone sodium phosphate.There was no difference in efficacy between the two administration methods for patients with sudden deafness of medium and high frequency descending type.
