摘 要:目的了解糖皮质激素受体(GR)基因多态性位点(N363S、ER22/23EK以及Bcl-1)与职业性三氯乙烯药疹样皮炎(OMDT)激素抵抗的相关性。方法选择2017年3月—2020年3月在深圳市职业病防治院职业病科住院治疗的OMDT激素敏感性和激素抵抗性患者各10例作为研究对象,选择同期进行健康检查者10例作为健康对照。采集研究对象外周血,采用PCR及一代测序进行GR基因多态性位点(N363S、ER22/23EK以及Bcl-1)检测。结果健康对照组3个单核苷酸多态性(SNP)位点均满足Hardy-Weinberg平衡检验。3组观察对象外周血GR基因N363S和ER22/23EK仅检测出AA和GG基因型,未检测出碱基变异。Bcl-1在3组外周血中均检测出3种基因型CC、CG和GG。健康对照组和GC敏感组的Bcl-1基因型GG及等位基因G频率较低,而GC抵抗组GG及等位基因G频率较高,但3组比较,差异无统计学意义(P>0.05)。结论 GR基因N363S和ER22/23EK未检测出多态性位点,Bcl-1基因多态性与OMDT激素抵抗可能不具有相关性,但尚需多中心大样本实验进一步探讨GR基因多态性与OMDT激素抵抗性的相关性。Objective To explore the relationship between glucocorticoid receptor(GR) gene polymorphism sites(N363 S,ER22/23 EK and Bcl-1) and glucocorticoid resistance in the patients with occupational medica-mentosa-like dermatitis due to trichloroethylene(OMDT). Methods Totally 10 OMDT patients with hormone sensitivity and 10 OMDT patients with hormone resistance were collected as the research objects in the Occupational Disease Department of Shenzhen Occupational Disease Prevention Hospital from March 2017 to March 2020. Meanwhile,10 people receiving the physical examination during the same period were selected as the healthy control. The peripheral blood of the study subjects was collected,and polymorphism sites of GR gene(N363 S,ER22/23 EK and Bcl-1) were detected by PCR and first-generation sequencing. Results All three single nucleotide polymorphism(SNP) sites in healthy control group met the Hardy-Weinberg equilibrium test. The genotypes of GR genes N363 S and ER22/23 EK from peripheral blood of the three groups were only detected AA and GG,and no base variation was detected. Three genotypes CC,CG and GG of Bcl-1 were detected from peripheral blood of the three groups. The G allele frequency and genotype GG of Bcl-1 were lower in healthy control group and GC sensitivity group,while the G allele frequency and genotype GG were higher in GC resistance group,but there was no statistically significant difference among the three groups(P>0.05). Conclusion No polymorphism sites were detected in GR gene N363 S and ER22/23 EK,and Bcl-1 gene polymorphism may not be correlated with OMDT hormone resistance. However,the correlation between GR gene polymorphism and OMDT hormone resistance needs to be further explored by multi-center large sample experiment.
