摘 要:目的探讨结核分枝杆菌mpt64 mRNA作为结核病化疗疗效监控的细菌学分子标志物的可行性。方法2021年1月至4月,于广州市胸科医院分枝杆菌菌种库中,随机选取35株对4种一线抗结核药物敏感的临床分离株进行培养,分别经异烟肼药物浓度为0、0.2、0.5μg/ml作用48 h后,提取其全RNA组反转录为cDNA,采用实时荧光聚合酶链式反应(PCR)检测mpt64 mRNA表达水平的变化。结果0、0.2、0.5μg/ml异烟肼药物浓度处理组在作用48 h后,35株结核分枝杆菌临床分离株的mpt64 mRNA Ct值分别为(25.34±2.56)、(29.09±3.25)、(30.14±3.23)。相较于无药对照组,0.2、0.5μg/ml异烟肼处理组mpt64 mRNA表达水平降低,差异有统计学意义(均P<0.05)。结论mpt64 mRNA表达水平可在短时间内反映出药物对结核分枝杆菌的作用效果,有望成为结核病化疗疗效监测的分子标志物。Objective To explore the feasibility of mycobacterium tuberculosis mpt64 mRNA as a bacteriological molecular marker for monitoring the therapeutic effect of tuberculosis chemotherapy.Methods From January to April,2021,35 strains of clinical isolates sensitive to 4 first-line anti-tuberculosis drugs were randomly selected from the mycobacterial strain library at Guangzhou Chest Hospital for culture.The isolates were treated by 0,0.2,and 0.5μg/ml isoniazid for 48 hours.The whole RNA group was extracted,reversed,and transcribed into cDNA.The mpt64 mRNA expression level was detected by real-time fluorescent polymerase chain reaction(PCR).Results After 48 hours'treatment,the 35 isolates'Ct value of mpt64 mRNA was(25.34±2.56)in the 0μg/ml isoniazid group,(29.09±3.25)in the 0.2μg/ml isoniazid group,and(30.14±3.23)in the 0.5μg/ml isoniazid group.The expression levels of mpt64 mRNA in the 0.2 and 0.5μg/ml isoniazid groups were lower than that in the 0μg/ml isoniazid group(both P<0.05).Conclusion The expression level of mpt64 mRNA can reflect the drug effect on mycobacterium tuberculosis in a short time,and may become a molecular marker for monitoring the efficacy of tuberculosis chemotherapy.
