四黄粉联合高锰酸钾对老年混合痔患者的疗效观察

摘　　要：目的观察评价四黄粉外敷联合高锰酸钾熏蒸坐浴对老年混合痔(MH)患者术后肛门水肿及疼痛的影响效果。方法选择2018年2月至2020年2月期间阳江市中医医院肛肠科收治的老年MH术后患者86例,随机对照试验分为两组,各43例。对照组男20例、女23例,年龄(67.12±6.79)岁;观察组男19例、女24例,年龄(66.97±7.01)岁。对照组给予1∶5000高锰酸钾溶液坐浴治疗,观察组则在对照组基础上给予四黄粉外敷治疗。比较两组患者治疗后肛门水肿、视觉模拟评分法(visual analogue scale,VAS)评分及术后首次排便时间、排便用时及切口愈合时间。结果两组术后第1 d肛门水肿评分比较差异无统计学意义(t=0.3084,P>0.05),观察组术后第3、5、7 d肛门水肿评分均明显高于对照组(t=4.3725、5.2847、5.4075,均P<0.0001)。两组术后第1 d VAS评分比较差异无统计学意义(t=0.4183,P>0.05),观察组术后第3、5、7 d VAS评分均明显低于对照组(t=3.1506、6.1367、8.6084,均P<0.001),观察组患者术后首次排便时间、排便用时及切口愈合时间均明显低于对照组(t=5.1664、7.1385、5.6720,均P<0.0001)。结论四黄粉外敷联合高锰酸钾熏蒸坐浴用于老年MH术后患者,能够显著地减轻其肛门水肿及疼痛程度,有效地促进手术切口愈合,值得临床大力推广应用。Objective To observe,evaluate,and analyze the effect of external application of Sihuang powder combined with fumigation sitting bath with potassium permanganate on postoperative anal edema and pain in elderly patients with mixed hemorrhoids(MH).Methods A total of 86 elderly postoperative MH patients who were admitted to Department of Anorectal Surgery,Yangjiang Hospital of Traditional Chinese Medicine from February 2018 to February 2020 were selected,and were divided into a control group and an observation group by the randomized controlled trials(RCT),with 43 cases in each group.There were 20 males and 23 females in the control group;they were(67.12±6.79)years old.There were 19 males and 24 females in the observation group;they were(66.97±7.01)years old.The control group took sitting bath with 1∶5000 potassium permanganate solution;in addition,the observation group externally applied Sihuang powder.The scores of anal edema and pain,the times for first defecation after the operation,defecation times,and incision healing times were compared between these two groups.Results One day after the operation,there were no statistical difference in the scores of edema and Visual Analogue Scale(VAS)between these two groups(t=0.3084 and 0.4183,both P>0.05).Three,five,and seven days after the operation,the scores of edema and VAS in the observation group were higher than those in the control group(t=4.3725,5.2847,5.4075,3.1506,6.1367,and 8.6084,all P<0.0001).The time for first defecation after the operation,defecation time,and incision healing time in the observation group were shorter than those in the control group(t=5.1664,7.1385,and 5.6720,all P<0.0001).Conclusion Sihuang powder combined with potassium permanganate for elderly patients with MH after surgery can significantly reduce their anal edema and pain,and effectively promote the healing of surgical incision,so it is worth being clinically generalized.
