摘 要:目的研究术前肌力锻炼对全膝关节置换术后早期功能恢复的影响。方法选取揭阳市人民医院2018年1月至2019年5月因严重膝关节骨性关节炎需要进行全膝关节置换术的40例患者,随机数字表法分为干预组和对照组,各20例。干预组中男性8例,女性12例,年龄(66.00±3.61)岁,术前4周进行肌力锻炼;对照组中男性10例,女性10例,年龄(67.00±2.56)岁,不进行术前肌力锻炼。比较两组患者术后股四头肌肌力恢复时间,采用患膝美国膝关节协会(KSS)评分和关节活动度(ROM)评估两组患者术后早期功能恢复情况。结果全膝关节置换术后,干预组股四头肌肌力恢复时间为(1.07±0.53)d,对照组为(1.96±0.24)d,两组比较,差异有统计学意义(P<0.05);术后1周、6周和3个月,干预组KSS评分分别为(80.0±5.1)分、(85.0±2.0)分、(93.0±4.3)分,对照组分别为(74.0±3.4)分、(79.0±3.7)分、(87.0±1.5)分,两组比较,差异均有统计学意义(均P<0.05);术后1周和6周时,干预组患者膝关节ROM分别为(98.00±4.70)、(109.00±8.80),对照组分别为(90.00±3.10)、(102.00±6.70),两组比较,差异均有统计学意义(均P<0.05)。术后6个月KSS评分以及术后3、6个月ROM,两组比较,差异均无统计学意义(均P>0.05)。结论术前肌力锻炼能加快全膝关节置换术后早期功能恢复。Objective To observe the effect of preoperative muscle strength training on early knee function recovery after total knee arthroplasty.Methods From January 2018 to May 2019,40 patients diagnosed with serious knee osteoarthritis who underwent total knee arthroplasty at Jieyang People's Hospitalwere divided into an intervention group and a control group by the random number table method,with 20 cases in each group.The intervention group had 8 males and 12 females,were(66.00±3.61)years old,and took muscle strength training 4 weeks before the operation;the control group had 10 males and 10 females,were(67.00±2.56)years old,and did not took the training before the operation.The muscle strength recover times of quadriceps were compared between these two groups.The Knee Society Score(KSS)and range of motion(ROM)were used to evaluate their early knee function recovery after the operation.Results After the operation,the muscle strength recover time of quadriceps was(1.07±0.53)d in the intervention group,and was(1.96±0.24)d in the control group(P<0.05).One and six weeks and three months after the operation,the scores of KSS were(80.0±5.1),(85.0±2.0),and(93.0±4.3)in the intervention group,and were(74.0±3.4),(79.0±3.7),and(87.0±1.5)in the control group(all P<0.05).One and six weeks after the operation,the ROM's were(98.00±4.70)and(109.00±8.80)in the intervention group,and were(90.00±3.10)and(102.00±6.70)in the control group(both P<0.05).There were no statistical differences in the KSS score 6 months after the operation and ROM's 3 and 6 months after the operation between these two groups(all P>0.05).Conclusion Preoperative muscle strength training accelerates the early knee function recovery after total knee arthroplasty.
