中药坐浴结合祛腐止血散对血液恶性肿瘤患者化疗后并发肛周脓肿的影响

摘　　要：目的探究中药坐浴结合祛腐止血散对血液恶性肿瘤患者化疗后并发肛周脓肿的影响。方法选取2018年5月至2020年5月期间收入济南市中心医院接受化疗的98例血液恶性肿瘤化疗后并发肛周脓肿患者为研究对象,按照随机数表法分为对照组与观察组,各49例。对照组男23例,女26例,年龄(38.65±8.65)岁;观察组男28例,女21例,年龄(39.25±9.01)岁。对照组采用常规干预,观察组在对照组的基础上叠加中药坐浴结合祛腐止血散干预。比较两组患者的炎性细胞因子水平以及临床症状改善情况。结果干预后,观察组肿瘤坏死因子-α为(16.69±4.21)μg/L、白介素-6为(93.98±9.63)ng/L以及白介素-8为(0.47±0.17)μg/L,而对照组分别为(14.52±3.19)μg/L、(102.98±8.35)ng/L、(0.58±0.15)μg/L,观察组显著优于对照组,差异均有统计学意义(t=2.565、4.409、3.030,均P<0.05)。观察组创面腐肉脱落时间为(3.54±1.63)d、创面愈合时间为(15.67±2.25)d、创面疼痛程度评分为(2.33±0.98)分、肛周脓肿面积为(0.11±0.03)cm^(2),而对照组分别为(4.98±1.35)d、(18.32±2.34)d、(3.87±1.12)分、(0.19±0.05)cm^(2),观察组改善效果显著优于对照组,差异均有统计学意义(t=4.603、5.121、6.462、8.568,均P<0.001)。结论血液恶性肿瘤患者化疗后并发肛周脓肿,应用中药坐浴结合祛腐止血散治疗,可改善患者症状体征,减轻炎性反应,值得临床借鉴。Objective To explore the effect of sitting bath with traditional Chinese medicine combined with Qufu Zhixue powder for perianal abscess in patients with hematologic malignancy after chemotherapy.Methods From May 2018 to May 2020,98 patients with perianal abscess after chemotherapy for hematologic malignancy in Jinan Central Hospital were selected as the study objects,and were divided into a control group and an observation group by the random number table method,with 49 cases in each group.There were 23 males and 26 females in the control group;and they were(38.65±8.65)years old.There were 28 males and 21 females in the observation group;and they were(39.25±9.01)years old.The control group was routinely intervened;in addition,the study group took sitting bath with traditional Chinese medicine and used Qufu Zhixue powder.The inflammatory factors and clinical symptoms were compared between these two groups.Results After the intervention,the levels of tumor necrosis factor-α,interleukin-6,and interleukin-8 were(16.69±4.21)μg/L,(93.98±9.63)ng/L,and(0.47±0.17)μg/L in the observation group,and were(14.52±3.19)μg/L,(102.98±8.35)ng/L,and(0.58±0.15)μg/L in the control group(t=2.565,4.409,and 3.030,all P<0.05).The time for the dead flesh to come off from the wound,wound healing time,wound pain degree,and perianal abscess area were(3.54±1.63)d,(15.67±2.25)d,(2.33±0.98),and(0.11±0.03)cm^(2)in the observation group,and were(4.98±1.35)d,(18.32±2.34)d,(3.87±1.12),and(0.19±0.05)cm^(2)in the control group(t=4.603,5.121,6.462,and 8.568,all P<0.001).Conclusion Sitting bath with traditional Chinese medicine combined with Qufu Zhixue powder for perianal abscess in patients with hematologic malignancy after chemotherapy can improve the patients'symptoms and signs and reduce their inflammatory response,so it is worth for clinical references.
