摘 要:目的探究应用强化驱铅方案治疗职业性铅中毒的近期效果及对患者实验室指标的影响。方法80例职业性铅中毒患者,按照抽签法将其分为对照组和观察组,每组40例。两组患者均进行对症治疗,对照组实施依地酸二钠钙治疗,观察组在对照组基础上加用水溶性维生素治疗。比较两组近期治疗效果,治疗前后铅含量指标水平,治疗前后实验室指标[红细胞新卟啉(ZPP)、T淋巴细胞(CD4+)、尿δ-氨基乙酰丙酸(δ-ALA)、尿视黄醇结合蛋白(U-RPB)]水平。结果观察组近期治疗总有效率95.00%高于对照组的77.50%,差异有统计学意义(P<0.05)。治疗后,两组血铅、尿铅水平均较治疗前降低,且观察组血铅、尿铅水平低于对照组,差异有统计学意义(P<0.05)。治疗后,两组ZPP、δ-ALA、U-RPB水平均较治疗前降低、CD4+水平较治疗前升高,且观察组ZPP、δ-ALA、U-RPB水平均低于对照组,CD4^(+)水平高于对照组,差异有统计学意义(P<0.05)。结论应用强化驱铅方案治疗职业性铅中毒具有比较好的近期疗效,能明显降低患者体内铅含量,改善相关实验室指标,降低对人体的损伤。Objective To investigate the short-term effect and laboratory indexes of intensive lead displacement program for patients with occupational lead poisoning.Methods A total of 80 patients with occupational lead poisoning were divided into control group and observation group according to lottery method,with 40 cases in each group.Both groups received symptomatic therapy.The control group was treated with disodium calcium ethylene diamine,and the control group was treated with water-soluble vitamins based on the control group.The short-term treatment effect,lead content indicators,laboratory indexes[zinc porphyrin(ZPP),T lymphocytes(CD4^(+)),urineδ-aminolevulinic acid(δ-ALA),urine retinol binding protein(U-RBP)]before and after treatment were compared between the two groups.Results The total effective rate of short-term treatment 95.00%of the observation group was higher than 77.50%of the control group,and the difference was statistically significant(P<0.05).After treatment,the blood lead and urine lead levels of the two groups were lower than those before treatment,and the observation group was lower than the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of ZPP,δ-ALA and U-RPB of the two groups were lower than those before treatment,and the CD4+level was higher than that before treatment;the levels of ZPP,δ-ALA and U-RPB of the observation group were lower than those of the control group,and the CD4+level was higher than that of the control group;all the difference was statistically significant(P<0.05).Conclusion Intensive lead-eliminating program has a good short-term effect on patients with occupational lead poisoning,and it can significantly reduce the lead content in patients,improve the relevant laboratory indicators,and reduce the damage to human body.
