枣仁安神胶囊对老年患者髋部骨折术后认知损害及谵妄状态的干预效果

摘　　要：目的观察枣仁安神胶囊对老年髋部骨折术后认知损害及谵妄状态的干预效果。方法选取2015年8月至2020年7月206例因髋部骨折行手术治疗的老年患者作为研究对象。将患者随机分为2组,进行为期2周的干预观察。对照组服用安慰剂胶囊,观察组患者服用枣仁安神胶囊。在术后(第1、3、7天)和干预2周时,采用意识模糊评估法(CAM)、简易精神状态量表(MMSE)和焦虑自评量表(SAS)评分,观察2组患者认知、谵妄等的改善情况。结果观察组术后认知损害发生率显著低于对照组(6.80%vs 30.1%,P<0.01),术后谵妄发生率也显著低于对照组(3.88%vs 26.21%,P<0.01)。观察组术后MMSE评分均显著高于对照组(P<0.05),同时术后CAM评分、SAS评分均明显低于对照组(P<0.05)。结论枣仁安神胶囊可显著改善老年髋部骨折患者术后认知,减少谵妄的发生,值得临床推广应用。AIM To observe the effect of Zaoren Anshen capsule in preventing and improving postoperative cognitive impairment and delirium in elderly patients with hip fracture.METHODS From August 2015 to July 2020,206 elderly patients with hip fracture surgery were selected as the research objects.According to the random number method,the patients were randomly divided into 2 groups and the intervention observation was lasted for 2 weeks.The control group was given placebo capsule,while the observation group was given Zaoren Anshen capsule.The cognitive judgment scale(MMSE),delirium rating scale(CAM)and self-rating anxiety scale(SAS)were performed on admission,postoperative day 1,3,7 and 2 weeks after intervention.The improvement of cognition and delirium in the 2 groups was observed and analyzed.RESULTS The incidence of postoperative cognitive impairment in the observation group was significantly lower than that in the control group(6.80%vs 30.1%,P<0.01),and the incidence of postoperative delirium in the observation group was also significantly lower than that in the control group(3.88%vs 26.21%,P<0.01).The MMSE scores of the observation group were significantly higher than those of the control group(P<0.05),while the CAM scores and SAS scores of the observation group were significantly lower than those of the control group(P<0.05).CONCLUSION Zaoren Anshen capsule can significantly improve the postoperative cognition of elderly patients with hip fracture and reduce the incidence of delirium,which is worthy of clinical application.
