摘　　要：目的:评价1565 nm M22-ResurFx非剥脱点阵激光(NAFL)联合异维A酸治疗痤疮的疗效和安全性。方法:回顾2015年-2017年该院皮肤科收治的60例中度痤疮患者,将其分为3组:单独口服异维A酸组(20例);单独使用1565 nm非剥脱点阵激光治疗组(20例);联合治疗组(20例):口服异维A酸联合1565 nm非剥脱点阵激光治疗3次。收集每次治疗前、后照片,并使用Visia和Antera 3D®系统评估治疗前、后患者痤疮的改善程度和治疗后不良反应。结果:治疗后3组患者痤疮均有不同程度的改善,其中以联合治疗组改善最为明显。Visia和Antera 3D®分析结果显示治疗后3组患者血红素、紫质及红色区等指标均有显著改善,其中以联合治疗组改善最为明显,与治疗前比较差异均有统计学意义(P<0.05)。所有患者均耐受治疗过程,疼痛评分为(4.00±1.18)分。不良反应包括不同程度的肿胀、红斑、结痂和色素沉着,2~14 d后自行消退。治疗中未观察到瘢痕、水疱及肝功能和血脂异常。结论:异维A酸联合1565 nm M22-ResurFx NAFL治疗痤疮有效,可减少不良反应,值得临床推广应用。Objective:To evaluate the efficacy and safety of 1565 nm M22-ResurFx non-ablative fractional laser(NAFL)combined with isotretinoin in the treatment of acne.Methods:Sixty patients with moderate acne from Department of Dermatology,Xijing Hospital,the Fourth Military Medical University from 2015 to 2017 were included.All subjects were divided into three groups according to the different treatment schedules:oral isotretinoin group(n=20),1565 nm NAFL group(n=20)and combined treatment group(n=20).The patients in the combined treatment group were treated with 1565 nm NAFL for 3 times,together with oral isotretinoin.Photographs were taken before and after each treatment.The improvement of acne and adverse reactions after treatment were evaluated using Visia and Antera 3D®radiography system.Results:The three groups of patients showed different degrees of improvement after treatment,and the most obvious improvement was found in the combined treatment group.The patients in combined treatment group had significantly improved indexes of acne degree(P<0.05)as compared to those in the other two groups.All patients tolerated the treatment and the average painful score ranged from 3 to 7 points.The treatments resulted in variable degrees of swelling,erythema,scab,and hyperpigmentation,but all these adverse reactions disappeared within 2-14 days after treatment.No scars or blisters were observed after the treatment.Conclusion:The combination of isotretinoin and 1565 nm M22-ResurFx NAFL is effective in treating acne with tolerable adverse reactions,and therefore is worthy of clinical promotion and application.
