摘　　要：硬脊膜动静脉瘘(SDAVF)是一类最常见的脊髓血管畸形疾病,其临床症状及影像学表现缺乏特异性。SDAVF在MRI中的典型表现是T2加权序列髓内高信号和髓周血管扩张,MR血管成像和CT血管成像可识别异常的动静脉结构、供血动脉和引流静脉,有助于疾病的诊断和治疗,未来或可替代DSA成为诊断此类疾病的新手段,但目前DSA仍然是诊断SDAVF的“金标准”。随着影像学技术不断进步,容积性T2 MRI、三维稳态进动结构相干、MR三维稳态进动快速成像、三维快速自旋回波MR序列以及多达640层的CT的出现,将无创性影像学检查的准确率提升到了新的高度。而利用术中血管成像技术可进一步提高手术成功率,降低晚期复发率。该文对SDAVF的影像学研究进行归纳总结,以加强对SDAVF的认识。Spinal dural arteriovenous fistula(SDAVF)is one of the most common spinal vascular malformations,of which the clinical symptoms and imaging findings lack specificity.The most common manifestations of SDAVF in MRI are T2-weighted sequence intramedullary hyperintensity and perimedullary vasodilation.MRA and CTA can identify abnormal arteriovenous structures and feeding arteries,as well as drainage veins,which are helpful for the diagnosis and treatment.They may replace DSA as a new rule for diagnosing such diseases in future,but now DSA is still the gold standard for diagnosing SDAVF.With the continuous advancement of imaging technology,the emergence of MRI sequences such as volumetric T2 MRI,3D-CISS,3D-FIESTA,3D-TSE and 640 slices of CT has brought the accuracy of non-invasive imaging examination methods to a new level.The use of intraoperative vascular imaging technology can further increase the success rate of surgery and reduce the rate of late recurrence.In order to strengthen the understanding of SDAVF,the paper made a summary of the latest imaging studies on this disease.
