中药凝胶贴膏剂中的常用药物及临床应用研究进展

摘　　要：凝胶贴膏作为新兴的透皮给药载体是中药现代化的重要组成部分。整理2009—2019年与中药凝胶贴膏制剂工艺及临床研究相关的文献145篇,就中药凝胶贴膏药物组成、剂型变更、临床应用3方面进行综述,总结适合做成凝胶贴膏剂的中药种类,分析适宜于治疗的病症类型。发现出现频率较高的中药多为富含挥发油的通经走络和气味俱厚的中药,如大黄(15.8%)、延胡索(15.2%)、冰片(13.8%)、当归(13.8%)、乳香(13.1%)等,并存在有毒中药和动物药的应用;凝胶贴膏用于癌痛、关节炎、骨折外伤治疗较多,儿科用药也比较适宜。原剂型为传统中药外用制剂的,如散剂、油膏、丸剂、洗剂、酊剂等多可改进为凝胶贴膏剂。还对凝胶贴膏剂产业化可能存在的问题进行探讨,以期为凝胶贴膏剂的开发利用提供参考。As a new transdermal drug delivery system,the cataplasma is an important part in the modernization of traditional Chinese medicine.After reviewing the composition,dosage form change,and clinical application of Chinese medicinal cataplasms based on 145 articles related to their preparation technology and clinical research published from 2009 to 2019,this paper summarized the types of Chinese herbs suitable to be prepared into Chinese medicinal cataplasma as well as their clinical indications.It was found that the frequently adopted Chinese herbs were mostly rich in volatile oils,effective in dredging meridians and collaterals,and strong in smell and flavor,taking Rhei Radix et Rhizoma(15.8%),Corydalis Rhizoma(15.2%),Borneolum Syntheticum(13.8%),Angelica Sinensis Radix(13.8%),Olibanum(13.1%),for instance.Some toxic herbs and animal medicines were also utilized.The prepared Chinese medicinal cataplasma were applicable to both adult and children patients with cancer pain,arthritis,fracture and trauma.The traditional dosage forms for external use such as powder,ointment,pill,lotion,and tincture could all be transformed into cataplasma.Besides,this paper also discussed the potential problems in the industrialization of Chinese medicinal cataplasma with a view to provide new ideas for their development and utilization.
