针药结合治疗多囊卵巢综合征合并未破裂卵泡黄素化临床研究

摘　　要：目的:观察针药结合治疗多囊卵巢综合征(PCOS)合并未破裂卵泡黄素化(LUFS)的临床疗效。方法:选取符合纳入标准的60例PCOS合并LUFS患者,随机分为2组各30例。西药组予克罗米芬及注射绒促性素治疗,针药组在西药组治疗方案的基础上加用补肾调周法中药及针刺治疗,2组均连续治疗3个月。期间观察比较优势卵泡、子宫内膜容受性,检测孕酮(P)、雌二醇(E_(2))、促黄体生成素(LH)含量,比较2组排卵率及妊娠率。结果:治疗3个月后,与治疗前比较,2组子宫内膜厚度明显增厚,E_(2)、LH、P升高,PI和RI明显减小,差异均有统计学意义(P<0.05);针药组子宫内膜厚度,E_(2)、LH、P含量均高于西药组,RI、PI均低于西药组,差异有统计学意义(P<0.05);排卵率针药组80.0%,西药组60.0%,妊娠率针药组56.7%,西药组36.7%,2组排卵率、妊娠率分别比较,差异均有统计学意义(P<0.05)。结论:针药结合治疗PCOS合并LUFS可促进子宫内膜及排卵功能恢复,提高排卵障碍所致不孕者妊娠成功率。Objective:To observe the clinical effect of acupuncture combined with medication for polycystic ovarian syndrome(PCOS)complicated with luteinized unruptured follicle syndrome(LUFS).Methods:A total of 60 cases of patients with PCOS complicated with LUFS in accordance with the inclusion criteria were selected and randomly divided into two groups,30 cases in each group.The western medicine group was treated with clomiphene and injection with chorionic gonadotropin;the acupuncture-medicine group was additionally treated with Chinese herbal medicine and acupuncture of Bushen Tiaozhou method based on the treatment of the western medicine group.Both groups were treated for three months.During the period,the advantage ovum and the endometrial receptivity were observed and compared;the contents of progesterone(P),estradiol(E2)and luteinizing hormone(LH)were detected;the ovulation rate and the pregnancy rate in the two groups were compared.Results:After three months of treatment,the endometrial thickness in the two groups was significantly increased when compared with that before treatment;E_(2),LH and P were increased;PI and RI were significantly decreased,differences being significant(P<0.05);the endometrial thickness and the contents of E_(2),LH and P in the acupuncture-medicine group were higher than those in the western medicine group,and RI and PI were lower,the difference being significant(P<0.05);the ovulation rate was 80.0% in the acupuncture-medicine group and 60.0% in the western medicine group;the pregnancy rate was 56.7% in the acupuncture group and 36.7% in the western medicine group,differences being significant(P<0.05).Conclusion:The therapy of acupuncture combined with medication for PCOS complicated with LUFS can promote the recovery of endometrial function and ovulation function,and improve the success rate of pregnancy in patients with infertility caused by ovulation disorder.
