摘  要:目的:对比研究X射线胸片与肺部超声在鉴别新生儿暂时性呼吸增快(TTN)和呼吸窘迫综合征(RDS)的应用价值。方法:选取医院收治的86例TTN及RDS新生儿的临床资料,依据临床诊断结果将TTN患儿纳入TTN组(54例),RDS患儿纳入RDS组(32例)。两组患儿均行肺部超声及X射线胸片进行诊断,对比两组患儿肺部超声与X射线检查结果的灵敏度、特异度和诊断准确率,分析两组患儿超声影像表现。结果:两组患儿以临床诊断结果为金标准,肺部超声诊断TTN的灵敏度、特异度、漏诊率、误诊率以及正确指数分别为90.74%、92.11%、7.89%、9.26%和0.829,X射线诊断TTN分别为83.33%、84.21%、15.79%、16.67%和0.675;肺部超声诊断RDS的灵敏度、特异度、漏诊率、误诊率以及正确指数分别为81.25%、93.33%、6.67%、18.75%和0.746,X射线诊断RDS分别为87.50%、96.67%、3.33%、12.50%和0.842。X射线诊断RDS特异度明显高于TTN,差异有统计学意义(χ^(2)=4.815,P<0.05)。结论:X射线与肺部超声均可有效诊断新生儿TTN和RDS,而肺部超声对TTN的诊断价值相对更高,X射线对RDS的诊断价值相对更高。Objective:To explore the application value of X-ray chest radiography and lung ultrasound in identifying transient tachypnea of newborn(TTN)and respiratory distress syndrome(RDS).Methods:The clinical data of 86 newborns with TTN or RDS admitted to hospital were selected,and they were divided into TTN group that included TTN newborns(54 cases)and RDS group that included RDS newborns(32 cases)according to the clinically diagnostic results.Both two groups underwent lung ultrasound and X-ray chest radiography for diagnosis,and the sensitivities,specificities and diagnostic accuracies of lung ultrasound and X-ray examination between the two groups were compared,and then the appearances of ultrasound images between the two groups were further analyzed.Results:According to the gold standard that was clinically diagnostic result,the sensitivity,specificity,missed diagnosis rate,misdiagnosis rate and correct index of lung ultrasound were 90.74%,92.11%,7.89%,9.26%and 0.829 in diagnosing TTN,and those of X-ray examination were 83.33%,84.21%,15.79%,16.67%and 0.675 in diagnosing TTN,respectively.And the sensitivity,specificity,missed diagnosis rate,misdiagnosis rate and correct index of lung ultrasound were 81.25%,93.33%,6.67%,18.75%and 0.746 in diagnosing RDS,and those of X-ray examination were 87.50%,96.67%,3.33%,12.50%and 0.842 in diagnosing RDS,respectively.The specificity that X-ray examination diagnosed RDS was significantly higher than the specificity that X-ray examination diagnosed TTN(χ^(2)=4.815,P<0.05).Conclusion:Both X-ray and lung ultrasound can effectively diagnose neonatal TTN and RDS,while the diagnostic value of lung ultrasound is higher in diagnosing TTN,and the diagnostic value of X-ray is higher in diagnosing RDS.
