超声联合区域神经阻滞对中青年胫骨骨折患者麻醉效果分析

摘　　要：目的:探讨超声联合区域神经阻滞对中青年胫骨骨折患者的麻醉效果。方法:选取在医院就诊的70例中青年胫骨骨折患者,依据麻醉方法的不同将其分为观察组和对照组,每组35例。观察组接受超声联合区域神经阻滞麻醉方法,对照组接受硬膜外神经阻滞麻醉方法,比较两组患者的麻醉效果和不良反应发生情况,评价视觉模拟量表(VAS)评分情况。结果:观察组患者的麻醉起效时间、术后睁眼时间和完全清醒时间显著短于对照组,差异有统计学意义(t=7.394,t=6.643,t=6.478;P<0.05);观察组患者的麻醉优良率显著优于对照组,差异有统计学意义(χ^(2)=6.293,P<0.05);观察组患者术后12 h、24 h和48 h的VAS评分显著低于对照组,差异有统计学意义(t=11.111,t=4.801,t=4.220;P<0.05);观察组患者不良反应发生率为5.71%,对照组为11.43%,两组相比差异无统计学意义(χ^(2)=0.729,P>0.05)。结论:超声联合区域神经阻滞对中青年胫骨骨折患者具有较好的麻醉效果,且安全可靠。Objective:To investigate the anesthetic effect of ultrasonography combined with regional nerve block on young and middle-aged patients with fractures of tibia.Methods:70 patients with fractures of tibia who admitted to hospital were enrolled.And they were divided into observation group who received ultrasound combined with regional nerve block anesthesia method and control group who received epidural nerve block anesthesia,with 35 patients in each group.The anesthesia effect,occurrence of adverse reaction and visual analogue scale(VAS)of two groups were compared.Results:The onset time,postoperative eye opening time and full awake time of observation group were significantly shorter than those of control group(t=7.394,t=6.643,t=6.478,P<0.05).The excellent and good rate of anesthesia of observation group was significantly better than that of control group(χ^(2)=6.293,P<0.05).The VAS scores of patients of observation group at 12 th h,24 th h and 48 th h after surgery were significantly lower than those of control group(t=11.111,t=4.801,t=4.220,P<0.05).There was no significant difference in the incidence of adverse reactions between observation group and control group(5.71%VS 11.43%)(χ^(2)=0.729,P>0.05).Conclusion:Ultrasonography combined with regional nerve block has good anesthetic effect on young and middle-aged patients with fractures of tibia and its safety is reliable.
