补肾活血方联合地屈孕酮治疗早期先兆流产合并绒毛膜下血肿疗效观察

摘　　要：目的研究补肾活血方联合地屈孕酮治疗早期先兆流产合并绒毛膜下血肿(SCH)的临床疗效。方法选取2019年3月至2021年3月肇庆市中医院妇产科诊治的60例先兆流产合并SCH患者,按随机数表法将患者分为对照组和观察组,每组30例。对照组患者给予地屈孕酮治疗,观察组患者在对照组治疗的基础上给予补肾活血方治疗,两组均连续治疗14 d。比较两组患者治疗效果、SCH面积和血清雌二醇与孕酮含量水平。结果观察组患者的治疗总有效率为93.33%,明显高于对照组的73.33%,差异有统计学意义(P<0.05);观察组患者治疗后的SCH面积为(0.63±0.28)cm^(2),明显小于对照组的(1.51±0.33)cm^(2),差异有统计学意义(P<0.05);观察组患者治疗后的血清雌二醇与孕酮水平分别为(1894.54±389.53)pg/mL和(35.49±8.51)ng/mL,明显高于对照组的(1256.41±257.69)pg/mL和(27.52±6.39)ng/mL,差异均有统计学意义(P<0.05)。结论补肾活血方联合地屈孕酮治疗能够有效改善先兆流产合并SCH患者的症状,减少绒毛膜下血肿面积,其作用与提高患者孕酮与雌二醇的水平有关。Objective To investigate the clinical effect of Bushenhuoxue decoration combined with dydrogesterone in treating the early threatened abortion complicated with subchorionic hematoma(SCH).Methods A total of 60 patients with early threatened abortion complicated with SCH from Department of Gynaecology and Obstetrics,Zhaoqing Hospital of Traditional Chinese Medicine were divided into control and observation group,with 30 patients in each group.Patients in the control group were treated with dydrogesterone,while those in the observation group were additionally treated with Bushenhuoxue decoration.All patents were treated with 14 d.The clinical effects,the area of SCH,and the serum concentrations of estradiol and progesterone were observed and compared.Results The total effective rate in the observation group was 93.33%,which was significantly higher than 73.33%in the control group(P<0.05).The dimension of SCH in the observation group was(0.63±0.28)cm^(2),versus(1.51±0.33)cm^(2)in the control group(P<0.05).After medication,the serum concentrations of estradiol and progesterone in the observation group were(1894.54±389.53)pg/mL and(35.49±8.51)ng/mL,respectively,which were significantly lower than corresponding(1256.41±257.69)pg/mL and(27.52±6.39)ng/mL in the control group(P<0.05).Conclusion Bushenhuoxue decoration combinged with dydrogesterone could improve the symptoms of the patients with early threatened abortion complicated with SCH,and its effect is related to increasing the patient’s progesterone and estradiol levels.
